乙方（受助监护人）：

残疾儿童康复救助项目是20__年省委、省政府和市委、市政府为民办实事项目之一，是改善残疾儿童状况的一项抢救性工程。为办好办实事这项惠民工程，明确责任，经甲乙双方充分协商，特订立以下协议：

一、甲方责任：

1、甲方同意接受（乙方）为本康复机构在训人员享受政府补贴，为其提供康复训练服务。

2、甲方根据省定点康复机构建设和服务标准的要求，为乙方孩子提供孤独症儿童康复训练时间为一学年（10个月）、每天训练课时6课时（每课时30分钟），其中每天要安排单训课和感统课至少各一节的康复服务项目。

3、甲方根据定点康复机构建设标准的要求，保证训练场所，教学设备的完善，建立健全安全规章制度，确保受助对象的人身安全，杜绝一切事故的发生。

4、甲方要建立健全受助对象的康复训练档案，内容包括全年康复计划、康复记录、康复小结以及康复前期、中期、后期的评估，填发《省委、省政府为民办实事“残疾儿童康复工程”项目救助卡》（以下简称《救助卡》），保质保量做好受助对象的康复训练。

5、甲方可适当收取食宿费，食宿费不能超过当地物价部门规定普通幼儿园食宿收费标准。若有违反视为放弃作为定点康复机构单位。应保质保量做好受助对象的康复训练，并提供受助对象康复训练票据和救助卡记录。

6、甲方协助乙方凭《救助卡》及票据到残联领取政府定额补助资金。

二、乙方责任：

1、乙方应向甲方提供“残疾儿童康复受助审批表”、受助对象和监护人的户籍证明、受助对象的残疾证或诊断证明、监护人的贫困证明、儿童康复救助项目协议书，经市级、县（区、市）级残联审核批准后，方可享受补贴。

2、乙方应定期送受助对象到甲方康复机构参加康复训练，并自觉遵守甲方有关规章制度，服从甲方人员管理。

3、乙方对受助对象的身体状况、精神状况、特殊疾病、特异行为及其它特殊情况，应如实告知甲方，在甲方遇到乙方的受助对象发生临时病情变化，并及时送往医院抢救时，乙方接到通知后须第一时间赶往现场处理。

4、乙方享有政府补贴为每人1500元/月康复训练费。乙方在训期间，除按照文件规定要求受训时间外，如需要加训，其费用应由乙方自理。

5、乙方在训时间为孤独症儿童、智力残疾儿童年度康复训练时间少于6个月不享受康复救助。康复训练满10个月以上方能享受政府全额补助。

三、康复训练期限：自年月日起至年月日止。

四、本协议双方盖章签字后生效，一式叁份，甲方、乙方、残联各一份。

实习前老师给我们布置了几个实习任务，那么这个教育实习就从这几个方面来谈谈自己的体会吧。

一、课堂教学

1、听课

9号去的学校，朱校长将自己带到了指导老师那，说是高一的英语组组长尤老师。一听是组长，有些忐忑起来，所幸见到老师和悦的样子给自己讲各种各样的事宜，渐渐便平静了心情。实习的第一天便是听课。这时候的听课与以往的听课应当不同的。可叹自己做了20xx年的学生，听了各色各样的老师讲课，或严谨或幽默，或深入浅出，或步步深入，习惯了听课只是记笔记，学知识，从来没有留心去观察老师上课的方法。尤老师讲课声音很宏亮，我坐在最后面还可以听得非常清楚。第一次听课听的便是习题的评讲。那些题目自己做倒是能做上，只是要自己讲出个所以然来却非常困难。尤其是那些什么先行词、介词、连词之类的术语，自己最是难以说清。再加上大学里学的英语多是欣赏阅读，很少涉及到句子结构语法分析。大二时讲的语法课是全英文的厚厚的一本，当堂课还知道讲的什么。课后全不知什么了，发现自己的英语知识只是零散的东一块西一片，没有形成整体的结构框架，不知道怎样串起来，也不知道哪里是重点。课后说与尤老师，他说都这样的，知识整合成结构系统都是做了教师以后慢慢整理出来的。自己听了稍微宽了心，以后再听课留意下老师的串讲。

后来的日子里还去听了张老师、孙校长的课。张老师年轻，讲课时课堂气氛很活泼，孙校长讲课细致，到底是比较有经验。开始以为高中上课也必须全英语的，后来发现其实并不一定都要英语的，适当的汉语解释有助于学生的理解。

2、作业批改

英语的作业主要是英语周报以及课课练的试卷。高中的课程很注重练习。每天都必须一定量的练习训练。英语作业大都是选择题，所以改起来不算麻烦。不过还是发现了自己改作业的缺陷，同样一个班的试卷，别人十来分钟就批完了，自己恁是要花上个把个小时。后来还是尤老师教了自己，原来批改作业也有方法讲究的。若将答案写在另一张纸上，ABCD答案与试卷答案的间隔一样，批改起来只需将不一致的划掉就可以了，这样批改起来效率果然可以高很多。批改时发现有些题目学生错的比较集中，甚至答案都错到一个选项上，说明那个题目很容易被误导到那个错选项上。这样讲评时便需要着重讲解。批改作业主要也就是为了了解学生的掌握程度，以及查漏补缺吧。

3、课堂教学

大概是第三周的时候，自己有幸终于走上了三尺讲台，开始紧张异常。这真实讲课可与微格教室里的不同，下面真真切切的有六十多双眼睛看着自己呢。我讲的是第三单元的Welcome to theunit.备课时从网上找了很多的课件，然后整理了半天。走到教室时学生齐刷刷都将目光投向了我，先是一小会的骚动，铃声响了教室也安静了下来。随着Good Morning的进入，自己渐渐不再紧张，只是讲英语时还是眼睛离不开备课笔记，终究还是对自己没有自信。高一的学生还很活跃，教他们讨论问题时都很快的讨论起来，只是还是很难主动自己站起来回答问题。任何时候点名回答问题还是必须的。本来预计将Welcome讲完将Reading也提下的，结果还是没赶上。课后老师说自己讲的还好，学生也挺配合我的，心里面甜丝丝的，倒是惠的实话给了自己一记。她说她坐在后面根本听不清的。自己觉得自己声音已经不小的，后面都听不清，看来若想做教师，这嗓音还是需要训练的。想来老师只是给自己鼓励才那样说的。

后来又讲了两节试卷评讲课，一节单词的讲解。讲习题时明显就发现了自己的不足，知道答案却难以讲解的明白。每个题目都得到字典上去寻求解释，还需要向老师请教如何讲解。很多很细微的知识点也要向学生解释清楚的。经过这几节课的实践，自己虽然还是难以把握讲解的方法，总算还是学得了一些。还有一个就是任务型阅读。这是高考新增的题型，高一也开始训练。说白了，任务型阅读主要就是mainpoint的概括，尤老师讲做任务型阅读先要看空，分析出应当填什么词性的单词，然后在原文中找出类似的表达，变换下也就能写出答案来了。特别要注意首字母的大小写。

讲课时时间的安排要紧凑，发现自己讲课时说话太慢，讲的内容少而杂乱，没有形成程序。以后还是要注重锻炼自己的速度，思维要敏捷，做事情要干练。

二、班主任工作

自己并没有实习班主任工作，不过从旁看了些指导老师做的班主任工作。班主任与学生既要亲切又不能失了尊重与敬畏。有一天尤老师将班上的每个人都找来谈了一番话，语重心长，学习生活上有什么困难，学习要注意怎样的方法，数学要重点抓，英语要多读多背之类的都一一给与建议。有个学生比较内向，一直郁郁寡欢的样子，尤老师劝解了半天也不开口讲话，这样的学生只得慢慢的多加开导，让他心情开朗起来才能愉快学习。

General Introduction

I assumed and fulfilled a sewage analysis & dispose project for Nanjing sewage treatment plant. This was my first practice to convert a laboratory idea to a commercial product.

In retrospect, I find myself standing on a solid basis in both theory and experience, which has prepared me for the Ph.D. program. My future research interests include: Network Scheduling Problem, Heuristic Algorithm research (especially in GA and Neural network), Supply chain network research, Hybrid system performance analysis with Petri nets and Data Mining.

3. 为了便于乙方资金正常周转，故甲方须在规定时间周同乙方进行一次结算，甲方不得以任何理由拖欠乙方的款项。
