五、双方义务与责任

1.甲方的义务与责任：

1.1 甲方应提供与评价项目相关的下列资料，并对所提供资料的可靠性和准确性负责：

1.1.1 建设项目总平面布置图、车间平面布置图。

1.1.2建设项目工艺流程简介和工艺流程图。

1.1.3原料辅料使用情况(组成、用量等)，生产工艺流程中存在化学物料名称和接触情况。

六、评价费用和支付方式
