1、标题

在请假条上方的中央位置，用较大的字体注明请假条”三个字，根据请假事由的不同，也可以将产假”两个字加上，注明产假请假条”也是可以的。

2、向谁请假

5、时间

落款写完之后，另起一行，在落款的正下方，注明时间，这个时间一般是提交请假条的时间，譬如我在今年11月10日写了请假条，在11月16日将请假条提交给了单位相关部门或是相关领导，那么日期一般写XX年11月16日”。
