甲方：
乙方：
根据 县委、县府针对农村配网升级改造的方针、政策，就乙方向供电公司申请，甲方受供电公司的委托，对乙方所辖社电力线路进行配网升级改造这一惠农工程。为了保证此项工程能够安全，顺利的实施，也为了能够早日完成国家实施这项工程的宏愿：尽快的让边远山区的村民用上光明电、优质电、廉价电、放心电，让众多的村民们能够感受和亲身体验到现代电气化产品带给人类的巨大优越性。因此，甲方在接受供电公司严格的资质审查与委托后，到乙方所在地进行了多次实地踏勘，现状以及当地风土人情的了解，再经过与当地镇政府分管此项工程的领导，当地供电所领导，农网办项目负责人，乙方负责该项工作的村干部等多次协商后，结合本工程的特殊性，同时结合县委、县府成立的农村电网改造领导小组的指导思想以及农网办的相关规定和制度，特订立本协议，甲方将抬杆打洞的电杆的二次转运、电杆组立、金具安装，线路架设、材料上下车、转运、拆旧等相关工作的辅助用工承揽给乙方，为明确相互责任，经甲乙双方协商一致，达成以下安全责任协议：
一、甲方责任

3、乙方在工程实施之前，应招集村民大会，将此次电网改造工程的宗旨，相关精神，要求及注意事项告知广大村民，特别是不能强迫施工队伍现场施工人员按照他们的意思去任意变换电杆或者拉线点位，更不得私自移动电杆和拉线位置。
4、根据现行电力线路的架设与安装标准和要求，也为了日后，所架设的电力线路能够安全，稳定的运行，所以，在电力线路其通道的砍伐上，应按照标准，保证线路边项与林木间的最低距离为 米。
5、乙方在施工过程中若出现安全事故，应对伤者及时全力救护和事故现场善后处理工作，同时第一时间(24小时内)报告甲方事故情况。
三、事故责任及事故处理
1、鉴于甲方就本工程已向保险公司投保，就事故费用处理达成以下协议：
1.1伤残事故的处理，乙方在施工时发生伤、残事故，需住院治疗者，甲方负责支付伤才在县级人民医院治疗的治疗费用。乙方负责伤者护理、生活、误工费及后续补偿费。
1.2死亡事故的处理，乙方在施工时发生死亡事故，甲方负责将保险公司赔付的死亡赔偿金一次性支付给乙方(以保险公司赔偿金额为准)，其他所用费用由乙方负责赔偿。
2、乙方负责事故人员入、出院手续和安抚工作。
四、协议价款
1、电杆坑洞，拉线坑洞以及地锚洞，按均价 元/个的标准，以实际竣工数量为准，据实结算，所有坑洞的深度必须达到供电公司各部门的验收标准。
2、电杆包括包立，不分长短，按均价 元/根的标准，以实际竣工数量为准，据实结算(8米居多，10米有小部分)。
3、电杆确需车辆进行二次转运的，由甲方现场确认。甲方负责车辆转运的运费，乙方负责电杆的上下劳务费。车辆运费视现场情况而定。
4、其实各项诸如材料上下车，材料转运，金具安装，线路架设，拆旧等各个工序上所要用的辅助工，按均价 元/个的标准，以实际用工数量据实结算。
五、款项支付
1、本协议载明，由甲方支付乙方的费用，按甲方与供电公司农网办约定的工程进度款支付办法和比例同步进行。
2、甲方与供电公司在工程进度款拨付之前，对供电公司要求的相关手续，证明等。乙方应本着理解与配合的角度出发，无偿提供给甲方。
当然，甲方也务必信守承诺， 待供电公司拨付的施工进度款一到账，就立即按约定支付给乙方。
六、违约责任
甲方双方任何一方的违约，都由违约方按照本协议总价款的比例赔付另一方。
七、本协议一式两分，经甲乙双方签字后生效。

篇18：农机安全生产监督管理责任合同书_合同范本
为了加强农机安全生产监督管理，防治和减少农机生产安全事故发生，保障人民群众生命财产安全，促进全县农村经济发展，特制定本辖区内农机安全生产监督管理责任合同书，由各乡镇农机监理员与所辖区的农机驾驶员双方签订，共同监督实施。具体合同内容如下：
一、各类农机驾驶（操作）人员必须服从各级农机监理部门和农机监理人员的指挥，必须配合农机执法人员的工作，农机执法人员依据《新疆维吾尔自治区农业机械安全监督管理条例》规定，有权对各类农机及驾驶操作人员进行安全检查监督，农机监理员在执法期间应出示执法证件。
二、各类农机驾驶员必须在所在乡镇（场）农机监理员的组织下，按照驾驶员审验规定的要求，参加一年一度的审验，审验合格后方可允许驾驶员与驾驶证件相符的农业机械。否则，由农机监理员根据《条例》第三十条规定对驾驶员处以50元以上200元以下的罚款。
三、各类农机驾驶员对购置的各类农牧业机械必须到西安农机监理站申请报户挂牌，对已登记报户的各类农牧业机械必须按照检验标准和技术要求进行保养维修，参加一年一度的检验，检验合格后方可允许各种作业，否则，有农机监理员根据《条例》第二十条规定对其车主及驾车人员处以50元以上200元以下的罚款。
四、各类农机及驾驶操作人员必须按所在乡镇（场）农机部门规定的时间进行检验或审验，超过期限除按规定交逾期补检、补审费外，还要对车主或驾驶员进行罚款处理。
五、各类驾驶员必须按照佳通法规和机械操作规程进行各种作业，绝不允许违章操作，坚决禁止“黑车非驾”、客货混装、超载超速、牵引架站人、叶子板坐人等违章行为，否则，农机安全监理部门及农机执法人员对以上违章人员实行重罚，并予以扣车处理。
六、遵守《草原法》，不得以任何理由非法开垦草场，支持退耕还林工作。
七、农机监理人员及各类驾驶操作人员都要遵纪守法，必须按照《中华任命共和国安全生产法》、《新疆维吾尔自治区农业机械安全管理监督管理条例》及有关农机法规政策行使职权和安全生产，杜绝各类农机事故发生，由于违章驾驶发生各种伤亡事故，由驾驶人员及车主负责，并按，《安全生产法》、《条例》规定追究有关责任人的行政责任，情节严重的要追究刑事责任。
八、各级农机安全监理部门及农机监理员必须依法行政，秉公执法。不得在执法过程中以权谋私、坚决禁止各种违纪行为。农机驾驶人员可随时向县农机监理部门和纪检、监察部门举报农机执法人员的危机行为，县农机监理部门可会同纪检、监察部门对违纪当事人进行查处，举报。
九、各级农机监理人员及各类农机驾驶员必须履行合同规定条款双方相互制约，共同遵守。

中国，一个传承了五千年、拥有悠久历史的国家，自古以来便将责任、担当列入民族传统美德中。在当今时代，不少人都是独生子女，父母们捧在手里怕摔了，含在嘴里怕化了，过着衣来伸手，饭来张口的日子，根本不知道自己的责任是什么，更不论担当起自己的责任了。由于父母们的过分溺爱，在家人“无微不至”的照顾下、呵护下成长，认为自己是家庭的中心，家中的一切都应该围绕着他们转。正是由于这种心理的存在，如今的中学生责任感不强，担当意识差，不清楚自己所扮演的角色，更不清楚自己的责任。

最主要的原因还是因为我们所处的生活环境。从小便在父母的溺爱中长大，没有责任感和担当意识，躲避自己的责任，这对我们的发展十分不利。所以我们必须改变这种请况，从零做起，勇于承担自己的责任，那是你会为自己的改变感到高兴。
担当是一种态度、一种责任，也是一种行为，面对家庭、社会、民族、国家赋予的使命;不躲避、不推诿、不迷失、坦荡荡，这是我们应具备的素质。同时，也只有勇于承担的人，才能使自己的潜质得到充分的发挥，才能在成长的途中创造属于自己的精彩。
