依据国家有关职业卫生的法律、法规和技术规范、标准，以及操作规程、职业病危害事故案例等，通过对拟评价项目的详细分析和研究，列出检查单元、检查部位、检查项目、检查内容、检查要求等，编制成表，逐项检查符合情况，确定拟评价项目存在的问题、缺陷和潜在危害,必要时可进行量化分析。

3.依据国家相关技术规范和标准的要求，通过现场检测和实验室分析，对拟评价项目作业场所职业病危害因素的浓度或强度以及职业病危害防护设施的防护效果进行评定。

三、进度要求

乙方应于合同签订后，在____年____月_____日前完成______________项目职业病危害现状评价报告书的编制工作。

四、文件份数

乙方应向甲方提供职业病危害现状评价报告书8份。

五、双方义务与责任

1.甲方的义务与责任：

1.1 甲方应提供与评价项目相关的下列资料，并对所提供资料的可靠性和准确性负责：

1.1.1 建设项目总平面布置图、车间平面布置图。

1.1.2建设项目工艺流程简介和工艺流程图。

1.1.3原料辅料使用情况(组成、用量等)，生产工艺流程中存在化学物料名称和接触情况。

1.1.4建设项目设备情况。

1.1.5 建设项目职业卫生防护设施：如通风除尘排毒系统设计性能、车间通风换气(全面通风换气量、空气调节及洁净区参数)、事故应急求援设施(紧急救援站的面积、冲洗装置等)、污水处理系统等。

1.1.6 职业卫生管理措施：生产制度、劳动定员、安全操作规程、应急救援预案、作业工人的个人防护、年度职业病防治计划、培训制度、健康监护制度、有害因素监测制度等。

1.1.7 卫生辅助设施：工作场所办公室、更衣室、盥洗室(盥洗水龙头个数)、浴室(淋浴器的个数)、宿舍、食堂的设置情况。

1.1.8其他资料：该项目职业卫生监测档案，职工健康监护档案等。

1.2 根据本合同规定向乙方支付评价费用。

1.3 为乙方到现场工作的人员提供相应的必要的个人防护装备。

2.乙方的义务与责任：

2.2 乙方对评价报告文件质量负责。

2.3 乙方对甲方提供的技术资料负责保密，未经甲方许可不得向第三方泄漏。

六、评价费用和支付方式

Good evening,I know its 3:00pm now,But I always what to say good evening.People always call me .I think if I have a choice.I would tell my parents I want to be called .Ok,its a joke.To be in this class is a wonderful thing.Because god has told me that we will be the NO.1 during the next 3 years.Fine, I shouldnt tell you this because god also has told me not to tell others.I like making friends with others a lot.So I think we will have a very good time together.But, dont expect me to tell you how to talk to god.Thank you.

我认为自己是一个认真的孩子;而在老师和家长的眼里，我是一个活泼好学的女孩;可在同学们的眼里我却是多变的：有时候是他们的好伙伴;有时候却是雷厉风行的大队长，但在有些时候，却成了热心的好帮手。

身为一名学生的我严格遵守学校的各项制度，在学校，我努力学习，懂得遵纪守法，热爱集体，助人为乐。在班上，我勤学善思，帮助同学，是同学们最好，最信赖的好伙伴。在学习上勤学善思，起到模范带头的作用。我的成绩也算名列前茅了，但我还是努力要求自己攀上学习的高峰。当我同学遇到学习上的困难时，我总是伸出“救援之手”来帮他们解开“锁”。老师所交代我的事情，我会不折不扣地去完成，与老师架起一座沟通的桥梁。

而身为乖孩子的我在家里，我总是会帮妈妈作些家务，例如：扫地，拖地，擦桌子······爸爸总是夸我是个好孩子。

荣誉虽然是一个人的证明，可也是一把双刃剑，她会使人迷失方向，但我并没有被这些难得的荣誉冲昏头脑，我把他们当作前进的动力。因此，我曾多次获得三好学生称号，可我却从不骄傲，也不会为此嫉妒别人。

而最令我骄傲的一次获奖，就是XX年，在张家口市一中举办的“美伦歌杯”拉丁舞蹈大赛中荣获了双人组一等奖的奖杯一个和伦巴组三等奖的铜牌一个。我的努力没有白费!在台下大家的赞扬声中，我告诫自己：不能骄傲，这只是个开始，要继续努力!

“尽最大的努力，做最好的自己”是我信奉的原则。作为一名班干部，我处处严格要求自己，事事以身作则，对老师交给的任务总是积极认真地完成，不推拖，不糊弄，协助老师管理好班级，是老师的得力小助手。一直以来都能全心全意为同学们服务，用自己热情开朗的性格影响周围的同学，只要有我的地方同学们都笑的合不拢嘴，一个个前仰后合。我的热心赢得了大家对我的充分信任，从中也让我感受到了快乐

我认为，学习好最主要的原因就是要勤奋。名人名言说的好：“天才是百分之一的灵感，百分之九十九的汗水。”“书山有路勤为径”刚刚上一年级的时候，我的成绩不是特别优秀，但是我从不气馁，因为没有人天生就是天才。我总是向别人请教，直到弄懂了为止。

我也觉得积累对于学习来说也是一种勤奋。学习要勤奋，这里的勤奋不仅只追求上进、热爱学习，还指积累。学习中，课外的积累与课内的东西一样重要，是必要的、不可缺的。世界上没有任何一条捷径能马上让你提高成绩，一飞冲天。读书时的好词好句可以积累，大自然里的知识可以积累。积累的好处有很多，比如我就深有体会。每次考试的作文，别人正不知如何下笔之时，而我大脑里浮现出许多平时积累的东西，让我顿时有了灵感。

别人在高兴的玩的时候我在预习课文。别人到睡觉时，我还在整理一天所学。为了能使我的成绩有所提高，我付出了很多努力。当然，我的努力有了成效!我的成绩终于有了突破。但我仍然不敢有丝毫放松，依然是勤勤恳恳，一回家就复习、预习，成了我学习的好习惯

我特别喜欢看书，书是我的精神食粮。我有一个很大的书柜，存了很多不同的书，我很喜欢看儿童文学和科学探秘的书。我看书的时候，几乎都忘记了时间，只要有时间，我一秒都不会放过。因此，还闹了不少笑话。阅读能给予我力量。

现在我正努力学习去实现未来的愿望。我对自己提出了做作业的七条要求，如先复习、改错、再做作业，然后要预习新课内容。每次期未考试会订出计划，达不到目标时找原因，总结经验，令学习成为成长动力。我对知识的理解和领悟力较强，课堂上总有我响亮清脆回答问题的声音，主动自信的我总成为同学们的楷模。

成绩代表过去，我的人生之路才刚刚起步，蜜蜂只有辛勤采蜜，才能酿出甜美的蜜糖，雄鹰只有勇敢的博击风雨，才能使自己的翅膀更加强劲有力。大浪淘沙，方显真金本色，风雪过后，更见青松伟岸。今后我会更加努力，为班级争光，为自己的人生添彩，不辜负老师和家长的期望和厚爱。

踏着清爽的秋风，吻着醉人的菊香，在这丰收的季节，我从千里之外的辽东来到了素有天堂美誉之称的杭州，跨进了梦寐以求的浙江商业学院的大门，开始了我的大学之梦的旅程。

当我踏进这所高等学府的大门时，看到老师们热情的手臂，温暖的笑容，听到新同学们的欢歌笑语和亲如弟兄姐妹般的问候，让我周身的血液开始沸腾，心跳也开始加速。这是为什么呢?因为那里就是我人生的一个最最重要的转折点，一个让我圆梦的地方!

读大学是我们每一个年轻人孜孜以求的梦想，这个梦从我的儿时就开始了。那时候虽然我还不懂得什么是理想、什么是奉献、什么是职责，可我羡慕科学家、羡慕作家、羡慕宇航员、羡慕一切有作为的精英干将。渴望自我将来也能够像他们一样，成为他们其中的一员，为伟大祖国的建设事业做出自我的贡献。历经十二年的中小学的读书过程，让我逐渐明白了一个道理，那就是：要想成功，一要有聪睿的头脑，优良的道德品质，二要有渊博的知识，丰富的学识。三是要有健康的体魄，宽旷的胸怀。最后就是要有上天赐予的机遇。

而机遇是给有准备的人准备的，这个准备就是第一、第二和第三的资料，那么大学就是我们做这个准备的最好地方。只有做好了第一、第二和第三的准备工作，我们才能在机遇到来之时，从容不迫地伸出我们的双手去迎接他、应对他，而被他所接纳、所包容。我们也就能在自我胜任的舞台上，演好自我的主角。以不负家乡父老的殷殷期望，老师的辛苦栽培。

愚者错失机会，智者善抓住机会，成功者创造机会，机会只是给准备好的人。机遇真是“神奇”，它能给“山穷水尽疑无路”的人带来“柳暗花明又一村”的喜悦;它能让商人散尽千金“还复重来”……“机遇”说起来很神奇，其实它经常出此刻我们的身边，而智者能发现它、利用它走向成功。愚人往往错过它却抱怨命运的不公平。其原因就在于机遇只偏爱有准备的头脑，有准备的头脑才能辨识和把握机遇。

很荣幸能够参加这次竞争上岗，并且有幸能够入围今天的面试环节。首先，我要感谢组织多年的关怀和培养，感谢局党组给我们提供竞争上岗的机会。再此，我先简要的做一下我的自我介绍。

我叫，今年xx岁，本科学历，业余爱好是体育运动，比如乒乓球，羽毛球，篮球等。我是xx年五月份参加工作，11月份入党，记得当时我也是通过社会招考的方式步入城管工作岗位的，那时招考人数是30人，很荣幸那时是以第一名的成绩成为一名城市管理者，当然身份是协勤，到xx年2月份由于工作突出，以及城市管理工作的需要，最终大家整体被转为正式员工，我想这是我人生的第一次转折点。

作为个人我从xx年至xx年连续七年被评为优秀工作者和多次获得优秀共产党员称号，并且分别于xx年和xx年，获得年度天津市城市管理综合执法工作先进个人和年度优秀城管文明执法队员的市级荣誉称号。

我的工作得到了领导和同志们的肯定，特别是从xx年我们通过考试身份从一名事业干部转为国家公务员，而且自己也被组织任命为综合执法局第一大队副队长，这更加激发了我的工作热情，我的性格特点是：干一行，爱一行，有冲劲有韧劲，工作不打折扣，干就要干好，就要争取最好的成绩!

我通过自己的率先垂范始终带领大家工作在第一线，为我们局这几年的全市综合考评连续取得第一名的成绩做出了应有的贡献。

我把这次任命作为我人生的第二次转折点，这使我从一名单纯的执法队员过渡到一名真正的城市管理者。

当然我想成绩属于过去，我们不能老是躺在功劳薄上停滞不前，目前我县新城建设进行的如火如荼，我们执法局作为新城建设的主力军，更要发挥自己的铁军作用，为新城建设添砖加瓦。作为我自己，我既是一个执法者也是一名新城的搬迁受益者，我家也是搬迁户。

我有责任也有义务管理好新城，这次竞争上岗，我把她当作自身综合能力的一次测试，无论成功与否，都不会影响我的工作热情。

我决心以这次竞争上岗为契机，加倍努力工作，充分发挥自己的聪明才智，使自己的工作迈向新的台阶!当然我更加恳切的期待着能得到各位领导和同事们的信任和支持，能给我提供这个展现自我能力的平台，来实现我的人生价值!
