第二条：_________甲方责任与义务

1、 甲方须对乙方进行英语程度测评，全部培训方案以该测评结果为依据。

2、 甲方根据乙方实际英语程度，为乙方量身定制学习方案。培训细节详见本协议之培训要素。

3、 甲方负责在教学期间所需的场地安排，若发生费用，本费用由甲方全部承担。

4、 甲方负责选派优秀师资进行本方案教学，同时根据乙方实际学习进度与效果对教师进行评估和管理。

5、 甲方承担教师授课的课酬。

6、 甲方负责提供教材，教材涉及费用由乙方承担。

7、 甲方负责安排培训测评，测评以每10次为单位进行，测评内容包括书面与口语两部分，全面判定乙方每10次学习效果。

8、 甲方负责根据乙方学习效果调整既定学习方案，效果评估在本方案结课时测试。

1、乙方须认真阅读并签署配合培训承诺书;认真执行既定培训方案，认真听讲，及时完成课后作业，全面配合甲方教学，以达到最佳培训效果。

2、本方案培训时间变化，无论什么原因，甲乙双方均须提前一天通知并针得对方同意后方可调整。

第五条：_________教学效果保证方式

1、 经甲乙双方商定，培训时间暂定每周星期日下午2：_________30至4：_________30，每周2小时;暑期必须集中每天来学校培训，培训方案另订。

2、 甲方选派岳阳新航道哈福外语学校优秀师资进行3P教学，并对教学效果进行评定。评定结果以乙方参加所在学校举行的任何一次中考或期考达到提高30分的英语成绩为准，评定标准为在哈福外语学校入学测试或由乙方提供最近一次所在学校英语测试结果(交甲方存档)的基础上经过本合同协议100小时培训后提高了30分;若乙方考试成绩低于此标准，甲方同意乙方继续接受甲方3P英语培训直至达到提高30分为止;或者退还未提高部分的学费。(未提高部分学费按每分900元计算);如超过30分，超过部分乙方同样须按每分900元奖励给甲方或介绍相当于奖励部分金额学费的学生参加甲方3P培训学习项目。如乙方在100小时以内在入学测试基础上达到或超过了30分;乙方不得以任何理由拒绝继续参加培训或要求退还学费。出现这种情况时，乙方必须承担违约责任，甲方有权拒绝退还所收取乙方的任何费用。

About MyselfMy name is Li Hua. I was born in the city of Dalian, Liaoning Province. I studied in Guangming Primary School from 1984 to 1990. Then I entered NO. 6 Middle School where I mainly learned the subjects like Chinese, mathematics, English, physics, chemistry and computer. I have been very fond of and good at English and computer ever since. Therefore I placed first in the school computer competition last year.
