教育部教师请假条类型

一、请假类型

1、事假学校实行五天工作制，并给予教师每周一晌自由支配时间，教师应尽量把个人事情安排在工作时间之外，确因具体事情调整不开的，可请事假。

2、病假一般性疾病不予准假，确因身体不适需住院或在家治疗的，凭区级及以上医院证明可请病假。

3、婚假教师初婚的凭婚姻登记证明可请婚假，期限不超过一周;再婚的凭相关证明请假，期限不超过两天。

4、产假教师计划内生育的可请产假，期限按上级文件执行;计划外生育的原则上不予准假。

5、引流产及绝育手术假教师引流产或做绝育手术的可请此假，期限最多不超过一周。

6、陪护假教师直系亲属因病住院的可请陪护假，请假期限视具体情况而定。

7、丧假教师直系亲属因病或其他原因故去的，可请丧假，请假期限为三天。周年时准假一天，三年时准假两天。

8、房屋修建假教师因居住需要而修建房屋的可请此假，期限不超过一周。

9、其他假除以上原因，教师需要请假的(如自学考试、职称晋升等)，可根据具体情况给予适当假期。

二、请假手续

10、教师请假需办理书面手续，填写“姜楼小学教师请假单”。

11、请假不超过三天的，本人自行调好课，事后补上;请假超过三天的，由教务处协调安排其他教师上课。

12、请假不超过一天的，由教务处批准;请假一天以上三天以内(含三天)的，由校长批准;请假三天以上一月以内的由中心校批准;请假一月以上的由中心校报教育局批准。

13、教师请假期满要向准假部门办理销假手续;请假期满需要继续请假的要办理续假手续。

14、教师请假理由要真实充分，准假部门负责人要合理使用准假权限，不准越权准假。

15、学校校长请假三天以上的要报中心校批准，并安排学校部门负责人代为处理学校工作;学校部门负责人请假要报校长批准。
