一年来，我校主要做了以下几方面工作，现汇报如下：

（一）积极探索，勇于改革，办学理念有创新

每一个学年伊始，面对新的工作任务、新的形势、新的困难，学校领导班子迎难而上，积极探索，勇于改革，办学理念有创新。冯绍欢校长带领行政班子和全体教职工认真学习现代教育管理理论，集思广益，因地制宜，自觉运用现代科学的学校管理理论指导学校的工作实践，积极探索切合本校实际的办学路子，修订了我校新一轮的办学思想，确定了我校治校基本方略，办学基本风格和办校主要特色。

学校实行层级管理，目标明确，思路清晰。在新一轮办学理念的指引下，我校把以改革用人制度、奖惩制度、考评制度来推进各项工作的创新；用先进的理论指导工作实践；构建自己特具有的校园文化高度人性化管理和艺体特色教育。并要求每一位教职工在学校办学理念的指引下有自己的工作理念和工作风格。

科学的办学理念的真正价值在于落实到全校师生的具体行为，为此，我想法设法通过层级管理抓好检查、督促和落实工作。现在，青莲中学正朝着“内强素质、外树形象、群策群力、创新创优”的方向发展，各项工作的管理正逐步走向人本化、民主化、制度化、规范化、科学化。着力做到与时俱进，深化了教育教学改革，全面推进素质教育。

（二）决策民主，管理科学，办学质量有提升

1、团结协作，形成合力，炼就务实型班子

在“领导”这方面，冯绍欢校长做好表率，注意“领”，做好“领头羊”，但更注重“导”，指导下属办好事。学校领导班子比较善于领导和团结广大教职工，能充分调动广大教职工的工作积极性，使他们主动地完成好学校布置的各项工作。

冯绍欢校长十分重视学校行政领导班子的建设，注重发挥其团结协作的作用，协调好学校行政之间、各部门之间、行政与教师之间、教师与教师之间、教师与学生之间等各种关系，把他们“扭成一股绳”、“握成一个拳”，形成合力，共同完成工作。在新型的学校管理理念指引下，我校行政班子能够做到各司其职、各负其责、团结协作、雷厉风行、真抓实干。平时乐于听取广大教职工的意见，班子会上能各抒己见，畅所欲言，且一旦形成决议后，能统一思想，坚决维护落实执行，在教职工中赢得了较高的威信。同时学校行政班子也善于团结广大教职工，注意调动广大教职工的工作积极性，群策群力地开展学校各项工作。目前，青莲中学已逐步形成了“服务、民主、廉洁、高效”的领导作风。

2、以身作则，培训，冯绍欢校长等学校主要领导分别作了《对照教师专业标准，争做师德师风楷模》、《教师如何提高课堂教学质量》、《浅谈初中学困生学习思维障碍及应对策略》、《学校特色的内涵及其创建的一般方法》等专题讲座。冯绍欢校长用自己的言行举止、师德规范、人格魅力潜移默化的影响老师，使老师学习意识、师德品质、奉献精神、合作能力均有很大提高。在浓厚的学习氛围下，目前，我校已逐步成为一个学习型组织、竞争型的集体。有13位教师20篇教学论文分别在国家、省、市教育教研部门组织的论文评比中获奖或教育杂志上发表。XX年中考，我校上市一中和南阳中学分数线的考生比去年大幅增加，各科平均分总体综合名次全县第5名，实现了学年初预定的教学目标。

一、上学年工作成就

(一)学校管理逐步规范

首先，各校管理制度在不断完善。在原有的渭北初中、渭河小学、李家营小学、渭河初中等的基础上，很多校随发展的需要补充了新的内容，例如：李家营小学的安全制度(应急预案责任制、协议书)很有特色，值得借鉴;朱家坪小学的先进集体评选办法;渭河小学的师德师风管理制度;梁家营小学的财务管理制度。

其次，学校管理工作逐步靠实、精细。比较突出的如王家营小学的教学管理制度要求明确，教师做的十分到位且有前瞻性，学校管理工作耳目一新、令人赞不绝口，也是质量得以提高的根本;李家营小学的奖罚制度落实的实、兑现及时;滩儿小学依平时工作制定量化考核细则，管理工作有很大起色;宋家湾小学也在逐步正规，不因小而粗疏，教师不因条件差而懈怠。

再次，能严格执行各项教育规章制度，依法办学，落实管理制度较到位，无违规办学现象。

(二)办学条件改善显著

一是全面按预期目标完成了各项工程。总投资58万元的渭河初中中央补助实施中小学危改项目工程于今年秋季竣工投用。总投资近30万元的水月坪小学社会援建项目工程于去年秋季竣工投用。几乎所有学校都投入了大量资金改善学校内部办学环境、硬化美化校园、购置课桌凳、文娱体育器材、电教设备、制作报栏、购置卫生桶。而较为突出的，师生贡献较大，发扬了勤俭建校的首数水月坪小学。总投资6万余元的田家洼小学转移支付危改项目于今年秋季投用。

二是各校自筹资金改善办学条件步伐较大。比较突出的有渭河初中抓住校舍搬迁新建的机会投资8万多元，新修了厕所、校墙、护河堤、硬化校园等。渭河小学投资近三万元新修办公室一间。苟家河小学新修了校门及围墙。梁家营小学翻新教室、办公室屋面等，校园面貌均焕然一新。

(三)教师成长步伐加大

一是通过以“八荣八耻”和“定西市师德50问”及自评等学习教育活动，提高了教师的思想政治素质。

二是积极参加各类业务进修与学习。根据计划，上学年共有小学教师19人次，初中教师12人次参加了继续教育培训。有30人参加各层次的在职学历教育，另有近7人次参加了各类培训班学习，教师的职业素养普遍有所提高。

三是学区组织了基本功比武、教师普通话讲演赛、新课改课堂教学展示、公开教学、观摩教学、教案作业展评等大型教研活动十余次，各校也不同程度地开展了校本教研活动，促进了教师教育科研的积极性，提高了教师的业务能力。

四是有3名教师被评为县级骨干教师，15名教师受到市、县、镇各级表彰奖励，教育活力进一步增强。

(四)教育质量提升迅速

在教学工作上，我们坚持很抓教学质量这条生命线不放松。基本思路是：第一学期抓常规、夯基础，第二学期抓教研、提质量。主要的作法是成立学区教研会，统一部署，带领全学区的教研教改工作。以学区教研会为龙头，以学校教导处、教研组为依托，以多种形式的教学调研为抓手，激励先进，督促后进，及时发现并纠正教学工作中存在的问题，扬长避短，加强整改提高。可以说教研会成立一年来，在提高教学质量上还是功不可没的。在今年全县小学毕业班语文、数学、英语竞赛中，数学获全县团体二等奖，列第二名，有两名学生获个人二等奖。语文列全县第4名，英语位列全县第11名，尤其数学科在全县引起了较大的反响。今年全县小学毕业班教学质量检测评价中，学区综合成绩名列全县第3名，二类区第一名，比去年全县12名，二类区第二名，有大幅度提高。语文及格率98.98%，名列全县第3名，二类区第1名(上学年第9名)。数学及格率84.69%，全县第二名，二类区第一名(上学年第16名)。综合均分54.43分，全县11名，二类区4名(去年26名)。渭河小学在中心小学中，居全县第二名，二类区第1名。成绩喜人，提前达到了预定计划目标。今年初三毕业会考，在全县二类区20所学校中，渭北初中以总分33.6分居第8名，渭河初中以总分26.67分居全县14名。学科合格率，渭北初中第11名，渭河初中第9名。其他各年级在学区组织的两次质量检测考试中也取得了不菲的成绩。

二、存在的主要问题

一是教育的持续发展后劲不足。主要制约因素是教育资源的配置不当，具体表现在：一方面教师结构不合理，队伍现状暴露出配置上的极端不均衡，部分学校教师数量不足，部分学校教师老龄化现象严重，全学区集中表现为英语与信息技术教师缺乏，且教师队伍不稳定;另一方面，教学投入不力，各校在办学主导思想上，受已形成的思维定势的影响，普遍重视校舍、校容校貌等基础设施条件的改善而忽视了对教学软环境的改善，实验室、图书室、教学仪器、器材用品、学习用具等教学资源得不到进一步充实与完善;还有就是教师教学业务能力提高的自觉性不够，年龄偏大的教师满足于既有工作现状，不思变革，年轻教师热衷于学历提高教育。总之，是相当一部分教师不能积极主动地适应新课程改革的要求，努力学习新理念、新方法，积极撰写教研论文，提升执教能力，向成就名师的方向奋斗。

二是教学管理亟待加强。在平时督导检查中发现很多学校在学校管理上有思想、有实招，但是教学管理工作落空，突出表现在一无教学管理制度和教研工作计划，二无检查组织、督导不力，三无量化考核与记载。对教师的教学行为缺乏明确的要求，对教学质量的提升没有具体的举措，教师的教学行为随意性大，教学工作处于自然生长的状态，学生的养成教育几近空白。

二、今后的工作思路

新学年全学区教育工作的总体思路是：以质量效益观为核心，以校园文化建设为依托，以教研活动为载体，抓常规、提质量、树新风，促进教育的均衡发展，努力构建和谐校园，提升办学水平，为学生的发展奠定良好基础，为服务辖区内人民群众脱贫致富提供智力支持，力争办人民满意的教育。

(一)、进一步树立质量效益观

通过大面积提升教学质量，带动素质教育上台阶，以学生掌握扎实的基础知识和基本技能为首要出发点，促进学生的全面健康发展。我们总的办学目标是在全县二类区学校中占据首要位置，在全县总排名中力争进入前3名，各小学在巩固今年已有的成绩的基础上力争薄弱校、薄弱年级，尤其是英语、综合等薄弱学科要有大的突破。在本学期期中六年级抽考中已暴露出了一些问题，有些学校的教学质量令人担忧，而在今年的六年级毕业统考中就集中表现出了综合科成绩决定一个年级、一个学校乃至全学区成绩的非常严重的问题。语文及格率98%，数学及格率85%，综合均分仅54分，。这个教训是非常沉痛的，必须引起各位校长、全体教师的高度重视。通过督导检查，我们发现在小学英语、综合科目的教学中首要的不是师资师能的问题，在的突出问题仍然是教学态度的问题。很多学校借口工作任务重，英语、常识课开起来了，但是没有正规的教案，有的甚至不布置也不阅作业，上课照本宣科，教师大念一遍，学生小念一遍，以为如此便可以了。形成这种现象的根源在于教学管理未纳入量化内容。我们要求，以后凡属二类区的学校至少文化课要有详实教案，常识课要备略案，技能课要有教学计划，三类区学校降低一个标准要求。作业，文化课、常识课都要有，只是量的多少不同而已。两所初中在教学质量上要力争实现跨越式发展。就今年的初三会考成绩来看，渭河初中主要是巩固率、参考率等的问题，渭北初中主要是综合课成绩的问题。两所学校要认真分析问题，仔细查找原因，及时制定对策，从管理策略和教学行为上再下功夫。所有教师都必须清醒地认识到我们的教育是基础教育，给学生一个坚实的知识基础，这是我们义不容辞的责任，也是渭河人民对我们的最大要求。我们必须报着对他们负责任的态度，努力工作。

(二)积极开展校园文化建设

这一思路是我们在上学期末就提出来的本学年工作重点之一，因条件不成熟未能及时实施，现在来看，我们提的不早，教育的要求也已明朗化。各学校要把校园文化建设、创建文明校园活动当作加强学校管理、进行师德师风教育、开展未成年人思想道德建设的载体来抓。通过这一活动的开展，进一步转变校风、教风、学风，为素质教育打造广阔的平台，使学生德智体美得到和谐、健康的发展。

1、学校管理要做到“细、严、实、活、特”

“细”就是管理要精细、规范，实现由粗放型向集约型的办学模式转化;“严”就是工作程序严格、规范，真正体现一依法治校、以规施教;“实”就是各项工作要从实际出发，想实招、办实事、讲实干、求实效，切忌哗众取宠，搞形象工程、面子做法;“活”就是要给学生以灵活的发展空间，创造活泼的育人氛围，使各类学生都得到应有的发展;“特”就是结合各自实际，走特色发展之路，在办学上有特色、管理上有亮点、充分彰显个性。“细、严、实、活、特”的目标就是从“三风”建设入手，创建“三园”学校(学园、乐园、花园)。

2、教师在每学年工作中要制定两个目标，即一个个人成长目标，一个教育成果目标。实现两点成就(个人发展或文化素质提高的成就，专业研究成果或教学成就)。

3、学生在校做到“五个一”，每年为学校栽一棵树或种一株花、每学期读一本好书、每月有一个进步、每周背诵一首诗歌、每天做一件快乐的事。通过这些活动，培养学生的人文精神与科学精神，开展未成年人思想道德建设。

4、学校总务工作要积极为校园文化建设创设有利条件，在办学软资源上多投入，如信息技术设备建设，实验室、图书室等功能室的装配与补充，教师、学生文化活动的开辟，学生学习、活动辅助用品用具的购置与投入等等。通过经费倾斜政策，为文明校园创建再立新功。

三、探索管理体制、用人机制改革，推动义务教育的均衡发展

目前，在我学区教育资源上存在着严重的不均衡现象，且教师队伍现状不稳定，要求调整工作单位的教师较多，这给我们的工作带来很大的压力。我们教师队伍的现状是总体超编，局部多寡不均。在这种情况下教师分配、调动，上报时我们不予考虑，而一些薄弱校又面临着关门的危险，导致管理跟不上，效益下滑。根据全县教育会议的精神，(1)开展“城镇支援农村，近郊支援边远，强校支援弱校”的工作，“以城川区各学校为主要责任方分别与农村中小学结对帮扶，建立起相互对应，相互固定的和作实体，实现理念共享、资源共享、成果共享”“积极尝试相邻学校联合办学的举措”;(2)川区一类学校要定期组织教学能手、优秀教师到三类区学校举办教学观摩和教育成果展示活动，活动重在优质常态课，带动教师队伍整体水平的提高;(3)大力推行川区教师支援山区教育，逐步建立川区中小学教师和管理人员到山区学校任教或管理服务制度，对于支教教师在通勤补助、年度考核、评优选先、职称晋升上给予优先考虑和照顾;(4)要在兼顾边远农村学校学生就近入学的基础上，逐步撤并一些生源少、效益差的学校。并实行年度考核改革，建立能上、能下、能让的用人机制，激发教师积极性，激活教育资源;(5)要加强校长任期目标管理，促使校长加强自身修养，不断提高能力，对经考核不胜任职务的要及时进行调整，这次督导检查中就发现有两所学校的工作几进瘫痪状态，管理人员要注意;(6)加强教师师德师风建设，坚持正面引导教育为主，对问题教师建立责任追究制;(7)有条件的学校要努力完成小学附设学前班，坚决杜绝学前班、一年级混合编班。

四、大力加强学生的养成教育和思想道德建设

要给学生灌输正确的礼仪常规、生活常规和学习常规，努力发掘学生的非智力因素，抓细节、经常抓、抓反复，做到处处皆教育、时时抓细节。

(一)甲方的权利和义务

1.甲方应及时向乙方提供与评估委托范围相关的文件和资料，并协助乙方向有关部门、单位或个人查询、抄录房地产估价所必需的资料。

2.甲方应配合和协助乙方现场查勘，并提供相应的方便。

3.根据项目建设实际需要，甲方有权及时调整评估工作范围和时间进度计划，向乙方下达评估工作计划和任务，并可提前1天通知和安排乙方到达指定地点开展评估相关工作。

4.甲方拥有本项目评估工作成果及相关评估资料的全部知识产权。

(二)乙方的权利和义务

1.乙方应遵守有关法律法规，遵循独立、客观、公正、合法等原则，信守职业道德，对评估结果的合法性、真实性、合理性承担相应的法律责任。

2.乙方应严格按照房地产评估工作程序办事，接受甲方对评估工作计划和任务的安排，在甲方的协助下开展现场查勘工作，及时向相关人员解释有关政策和评估报告内容，对有异议的评估报告做出书面解释或更正。

3.无特殊原因，乙方工作人员应在甲方预约的时限内到达指定地点开展评估相关工作。在现场查勘时，乙方应一次性向相关人员提出评估所需的资料清单并在取得评估所需资料后2天内出具分户评估报告初稿，初稿经征求甲方意见后2天内出具评估报告修正稿，修正稿报甲方审(复)核后3天内出具正式报告。

4.由于甲方或第三方的原因，影响乙方估价工作进度或致使工作停滞，由甲方负责组织相关单位及工作人员及时解决、排除影响乙方正常工作的不利因素，乙方完成工作的最后时限按照因此拖延的时间进行相应顺延，乙方不承担延期责任。

5.乙方应按约定的时间将工作成果及相关资料移交甲方，乙方应对该工作成果及其相关资料承担保密义务，未经甲方同意，不得将该工作成果及其相关资料出卖、出借、允许给他人使用或自己挪作他用，并尽保密之责，不得向第三人透露。

6.乙方对其工作人员在合同履行期间的一切行为负责，期间发生的一切安全事故责任及因此发生的人身损害赔偿和其他费用由乙方承担，与甲方无关。

第五条 违约责任

1.甲、乙双方务必共同遵守本合同条款。若非乙方原因导致在乙方出具估价报告书前提出终止本合同的，甲方应支付乙方相应的评估费用;若因乙方自身原因导致甲方在乙方出具估价报告书前提出终止本合同的，甲方不支付乙方评估费用。

2.非甲方原因，乙方未能按合同约定时限提交评估报告，乙方应向甲方支付违约金，违约金从本项目评估服务费用中扣除，暂定按每份评估报告涉及的评估费用额百分之一(1%)每日累计。

3.乙方违反有关法律法规和合同约定，或不信守职业道德，或存在不恰当和腐败行为的，并且造成严重后果的，甲方可解除本合同，且乙方承担由此而引起的法律责任，同时赔偿由此造成的经济损失。

4.甲方无故终止合同，应按实际情况向乙方支付已完成评估工作的评估服务费，并向乙方支付本项目未完成评估服务费用20%的违约金。

5.乙方出具的估价报告经甲方复核，多次被鉴定为不合法或不规范或不合理且乙方不配合进行修改的，甲方有权终止本合同。

第六条 本合同未尽事宜，双方可协商解决，协商不成的，任何一方可依法向深圳市龙岗区人民法院提起诉讼。

九、乙方的责任：

十、违约处理：

1、认真制定安全生产计划。

在今年年初公司领导结合工程实际情况，针对本单位施工人员流动分散、野外作业、施工点多面广、机械设备多、工种多、危险性大等特点，首先制定了公司20xx年度安全生产管理目标，围绕全年安全生产管理目标，制定了详细具体的安全工作计划、安全措施。

2、施行安全管理目标。

公司建立了安全组织领导机构，成立了安全生产领导组。并成立了安全生产办公室，设专职安全员一名，负责公司安全生产的管理工作。各项目经理部、施工分队以及施工班组均建立了相应的安全生产领导小组，并制定了各项安全管理措施。形成一把手亲自抓，分管领导、专职安全员具体抓的格局，形成一级抓一级，一级保一级，一级对一级逐级负责的安全生产目标管理体系和管理网络。

三、存在问题和今后建议

一年来，通过公司全体干部职工的共同努力，做了大量的实际工作，安全工作取得了比较好的效果，全年安全无事故。但兄弟单位尤其和先进单位相比，我们无论是安全的软件上还是硬件还存在一定的差距，主要表现在以下几个方面：

1、在各项制度的建立与实际管理操作上，有个别施工分队还存在或多或少的“三重三轻”现象;

2、施工现场安全管理不到位，存在着侥幸心理。例如危桥改造施工中，由于受场地限制，作业现场与交通道路界线不清。安全标志标牌设置不规范，管理不到位，被盗后不能及时重新设置。个别分队现场有些零乱，存在着事故隐患;

3、由于今年工程任务紧，工程量大，机械设备利用率高，机械维修保养有时不及时，存在着带病作业的现象，比如不及时换机油及空气虑芯器、制动不灵等。特别是摊铺分队工程任务紧，工程量大，机械维修保养不及时，安全意识不够，安全检查不到位;

4、安全台帐记录不到位、安全活动不够正常;

一、责任范围

本保险分为平安险、水渍险及一切险三种。被保险货物遭受损失时，本保险按照保险单上订明承保险别的条款规定，负赔偿责任。

（一）平安险。本保险负责赔偿：

1、被保险货物在运输途中由于恶劣气候、雷电、海啸、地震、洪水自然灾害造成整批货物的全部损失或推定全损。当被保险人要求赔付推定全损时，须将受损货物及其权利委付给保险公司。被保险货物用驳船运往或运离海轮的，每一驳船所装的货物可视作一个整批。推定全损是指被保险货物的实际全损已经不可避免，或者恢复、修复受损货物以及运送货物到原订目的地的费用超过该目的地的货物价值。

2、由于运输工具遭受搁浅、触礁、沉没、互撞、与流冰或其他物体碰撞以及失火、爆炸意外事故造成货物的全部或部份损失。

3、在运输工具已经发生搁浅、触礁、沉没、焚毁意外事故的情况下，货物在此前后又在海上遭受恶劣气候、雷电、海啸等自然灾害所造成的部份损失。

4、在装卸或转运时由于一件或数件整件货物落海造成的全部或部份损失。

5、被保险人对遭受承保责任内危险的货物采取抢救、防止或减少货损的措施而支付的合理费用，但以不超过该批被救货物的保险金额为限。

6、运输工具遭遇海难后，在避难港由于卸货所引起的损失以及在中途港，避难港由于卸货，存仓以及运送货物所产生的特别费用。

7、共同海损的牺牲、分摊和救助费用。

8、运输契约订有“船舶互撞责任”条款，根据该条款规定应由货方偿还船方的损失。

（二）水渍险。除包括上列平安险的各项责任外，本保险还负责被保险货物由于恶劣气候、雷电、海啸、地震、洪水自然灾害所造成的部份损失。

（三）一切险。除包括上列平安险和水渍险的各项责任外，本保险还负责被保险货物在运输途中由于外来原因所致的全部或部分损失。

二、除外责任

本保险对下列损失不负赔偿责任：

（一）被保险人的故意行为或过失所造成的损失。

（二）属于发货人责任所引起的损失。

（三）在保险责任开始前，被保险货物已存在的品质不良或数量短差所造成的损失。

（四）被保险货物的自然损耗、本质缺陷、特性以及市价跌落；运输迟延所引起的损失或费用。

（五）本公司海洋运输货物战争险条款和货物运输罢工险条款规定的责任范围和除外责任。

三、责任起讫

（一）本保险负“仓至仓”责任，自被保险货物运离保险单所载明的起运地仓库或储存处所开始运输时生效，包括正常运输过程中的海上、陆上、内河和驳船运输在内，直至该项货物到达保险单所载明目的地收货人的最后仓库或储存处所或被保险人用作分配、分派或非正常运输的其他储存处所为止。如末抵达上述仓库或储存处所，则以被保险货物在最后卸载港全部卸离海轮后满六十天为止。如在上述六十天内被保险货物需转运到非保险单所载 明的目的地时，则以该项货物开始转运时终止。

（二）由于被保险人无法控制的运输延迟、绕道、被迫卸货、重行装载、转载或承运人运用运输契约赋予的权限所作的任何航海上的变更或终止运输契约，致使被保险货物运到非保险单所载明目的地时，在被保险人及时将获知的情况通知保险人，并在必要时加缴保险费的情况下，本保险仍继续有效，保险责任按下列规定终止。

1、被保险货物如在非保险单所载明的目的地出售，保险责任至交货时为止，但不论任何情况下，均以被保险货物在卸载港全部卸离海轮后满六十天为止。

2、被保险货物如在上述六十天期限内继续运往保险单所载原目的地或其他目的地时，保险责任仍按上述第（一）款的规定终止。

四、被保险人的义务

被保险人应按照以下规定的应尽义务办理有关事项，如因未履行规定的义务而影响保险人利益时，本公司对有关损失，有权拒绝赔偿。

（一）当被保险货物运抵保险单所载明的目的港（地）以后，被保险人应及时提货，当发现被保险货物遭受任何损失，应即向保险单上所载明的检验、理赔代理人申请检验，如发现被保险货物整件短少或有明显残损痕迹应即向承运人、受托人或有关当局（海关、港务当局等）索取货损货差证明。如果货损货差是由于承运人、受托人或其他有关方面的责任所造成，并应以书面方式向他们提出索赔，必要时还须取得延长时效的认证。

（二）对遭受承保责任内危险的货物，被保险人和本公司都可迅速采取合理的抢救措施，防止或威少货物的损失，被保险人采取此项措施，不应视为放弃委付的表示，本公司采取此项措施，也不得视为接受委付的表示。

（三）如遇航程变更或发现保险单所载明的货物、船名或航程有遗漏或错误时，被深险人应在获悉后立即通知保险人并在必要时加缴保险费，本保险才继续有效。

（四）在向保险人索赔时，必须提供下列单证：

保险单正本、提单、发票、装箱单、磅码单、货损货差证明、检验报告及索赔清单。如涉及第三者责任，还须提供向责任方追偿的有关函电及其他必要单证或文件。

（五）在获悉有关运输契约中“船舶互携责任”条款的实际责任后，应及时通知保险人。

五、索赔期限

本保险索赔时效，认被保险货物在最后卸裁港全部卸离海轮后起算，最多不超过二年。

（i）scope of cover：

this insurance is classified into the following three conditions- free from particular average （f. p. a. ）， with average （w. a. ） and all risks. where the goods insured hereunder sustain loss or damage ， the company shall undertake to indemnify therefor according to the insured condition specified in the policy and the provisions of these clauses ：

1. free from particular average （f. p. a. ）.this insurance covers ：

1）total or constructive total loss of the whole consignment hereby insured caused in the course of transit by natural calamities： heavy weather， lightning， tsunami， earthquake and flood. in case a constructive total loss is claimed for， the insured shall abandon to the company the damaged goods and all his rights and title pertaining thereto. the goods on each lighter to or from the seagoing vessel shall be deemed a separate risk. constructive total loss refers to the loss where an actual total loss appears to be unavoidable or the cost to be incurred in recovering or reconditioning the goods together with the forwarding cost to the destination named in the policy would exceed their value on arrival.

2）total or partial loss caused by accidents the carrying conveyance being grounded， stranded， sunk or in collision with floating ice or other objects as fire or explosion .

3）partial loss of the insured goods attributable to heavy weather， lightning and/or tsunami， where the conveyance has been grounded， stranded， sunk or burnt. irrespective of whether the event or events took place or after such accidents.

4）partial of total loss consequent on falling of entire package or packages into sea during loading， transshipment or discharge.

3. all risks. aside from the risks covered under the f. p. a. and w. a. conditions as above， this insurance also cover all risks of loss of or damage to the insured goods whether partial or total， arising from external causes in the cause of transit.

this insurance does not cover ： .

1. loss or damage caused by the intentional act or fault of the insured.

2. loss or damage falling under the liability of the consignor.

（iii）commencement to termination of cover：

1. warehouse to warehouses clause ：

小学教育教学评估整改报告

XX年3月11日，区小学教研室主任周广忠一行3人来到我校，对我校――－XX学年度的教育教学工作进行全面评估和指导。领导们专业、细致、热情的指导，使我们如浴三月的春风，既温暖又喜悦。使我们在看到取得成绩的同时，也清醒地意识到工作中存在的问题、不足；现将此次评估情况总结如下：

一、评估工作的整体情况：按此次评估的流程，评估组一行首先分别听了语文、数学、综合实践三节课，然后与授课老师、全镇的主任一起进行的评课。授课老师们的认真备课、激情授课得到了领导们的高度认可；档案资料归档较为规范、齐全，也受到了领导们的一致肯定，总的来说符合上级的要求，达到了此次评估、督导的目的。

二、存在的问题。

第一、从档案的归整、积累方面：1、档案皮应采用是侧开式的，这样不利于工作人员的检查；2、档案皮封面的个别书写题目与内容一致，导致形式与内容的脱节；3、档案资料分散，不系统，没有实现模块化摆放，并且也没有摆放相应的提示标牌；4、平时活动谋化不足、设计不细，过程性资料积累偏少；

第二、在教学方面，特别是在高效课堂目标达成方面，也存在一些亟待改进和提高的地方：1、在备课环节，老师们没有充分参研课程标准、没有吃透教材，对学情的分析不够，课堂各个环节不够合理，设计目的不够明确和突出，导致教师教的质量差、学生学得效果低；2、没有很好地按区小学教研室下发的“目标导学”的相关资料去设计、去执行，更谈不上结合学情和本人的教学情况进行再设计了。

第三、校本课程和综合实践课程的开发和利用还不够突出，体系还不够完善，资料的规整还不够完善。

针对上述存在的问题，我们进行了认真、全面、深刻的反思，决心全面梳理教育教学思路，整改和提高我们的教学行为：

第一、从档案的归整方面：一律更换和使用国标的全开式的档案皮，封面书写规范，书写题目必须与内容完全相符，实行整齐划一式管理；档案资料的整理与存放实行模块化、系统化，与细则内容一一相对，并且设立专门醒目的标牌，以方便检查与评估；平时在开展教育教学活动时，要提前谋化，设计具体、详细的方案，争取实现活动的开展既有利于提高老师业务水平，又能提高学生的素质，又能积累下详实的资料，达到“一箭多雕”的效果，要把活动的开展、资料的积累放在平时，实现正规化、常态化，真正做到早准备、真实施，全归档。

第二、在高效课堂的建设方面：

1、抓好备课关。

教材、课程标准是我们进行教学的根基，如果在没有充分参研课程标准、没有吃透教材的基础上进行的备课是盲目的，课堂教学必定的低效的、甚至是无效的，所以必须严把备课关，精心设计每一个环节，突出目的性，牢牢以学生为中心进行，实行主体教学、目的备课。

2、在自己没有更好的教学思路的情况下，一定要先认真学习，研究小学教室下发目标导学，先去执行、去实践，然后能时间成熟再去再设计、再改良，这是今后我们高效课堂建设的基本思路和基本要求。3、在校本课程与综合实践开发与利用方面，组织老师们学习有关校本课程与综合实践方面的相关理论知识，并且与平时的教育教学活动相结合，做到每个年级有主题，每班的各个小组有子课题，开展活动先有计划，后有总结、活动评价，中间有手抄报、作文、心得等各种过程性资料，形成一个完整的体系，实现与教材内容的有机衔接与相互补充，发挥课程的真正目的和价值。

作为一个北部山区的学校，我们虽然面临这样的困难和那样的挑战，但这决不能成为我们搞不好教育教学的理由。我们一定要以这次活动为契机，以“目标导学”为指针，努力学习，锐意创新，打造丰润偏远山区的高效、规范的学校。
