二、评估内容

根据学校教学工作中影响教学质量的最直接、最核心的有关要素和环节，共设立“教师队伍建设、教学管理、教科研工作、特色特长教育、保障体系、考测与成绩管理”六个一级指标和十九个二级指标，并把二级指标分解为四十七个三级指标进行评估。三级指标对学校教育教学工作提出相关要求，力求使评价做到全面、准确、完整、公正和合理，并便于操作和比较。

三、评估方法

1、采用加权评分办法，以三区文教体局、全市小学、初中、普通高中为评估对象，三个学段都有相应的评估内容和分值，满分为1000分。

2、采用直接评估和抽查评估两种方式。对区教育行政部门及业务部门、市直属学校采取直接评估的方式，由市教体局全面评估;对区属学校采取抽查评估的方式，抽签定校，其学校所得的分值算作各区的分值。评估结果分类按照位次管理的办法在全市予以通报。

3、评估内容的有关信息主要通过听汇报、召开座谈会、问卷调查、查阅档案资料(特别是原始资料)、听课、验证、统计等方法采集。凡涉及指令性任务和师生竞赛获奖情况、考测成绩等以市教体局或相关科室(馆、中心)提供的统计数据为准。对某些工作的具体要求，亦以市教体局或相关科室(馆、中心)的要求为准，计算方法以市教体局确定的计算方法计算。

一、指导思想

第一阶段：(2月18----29日)学习动员，调查摸底

1、学习动员。学习教育局有关文件，制定《槟榔小学关于迎接20xx年教育教学督导工作的实施方案》，明确工作方向，明晰工作思路，把握工作重点，确定工作职责。召开学校迎检动员会，深入进行宣传发动工作。

2、调查摸底。对照标准，对学校办学条件图书、音体美劳卫器材等方面的缺口进行自查并分类汇总。

第二阶段：(3月---6月)集中准备，落实提高

1、加大投入力度，实现办学条件标准化。学校要借市督导评估之契机，多渠道筹集资金，努力实现办学条件标准化。

2、加强校园文化建设。一是绿化美化校园：粉刷墙面，油漆墙裙和栏杆，栽种花草。二是墙面文化，包括墙面文化、宣传橱窗等。三是班级文化建设，丰富班级文化内涵。

我校始建于1976年，是一所城市独立初级中学，XX年6月、XX年8月，市上对初中教育资源两次进行整合，把原来市二中、市四中与xx中学整合为新的xx市第二中学。现校址为前河沿路284号。学校有专任教师262人，60个教学班，学生2797人，其中住宿生328名，是xx市唯一一所给初中学生提供食宿的学校。近年来上级主管部门、学校一直把改善办学条件，优化育人环境作为普及教育的奠基工程来抓，通过努力目前我校办学条件已基本适应义务教育的要求，现将我校义务教育阶段办学标准情况报告如下：

（一）核心指标

1.学校设置与班额

根据州、市布局规划安排，我校面向10个城市小学、进城务工的外县流动人口子弟及辖区招生，现有学生2797人，班级60个，平均班额为46 .6人，无超大班额现象。

2.设备设施和场地

学校占地面积27972平方米，生均10平方米。现有250米和200米的环形跑道两个、篮球场六个、排球场六个（均是水泥场地）、乒乓球台10个、化学实验室4个、物理实验室6个、生物实验室2个，物、化、生仪器配备均未达到标准要求；多媒体音乐教室2个，教学设施只有一架钢琴和一架电钢琴、美术教室1个，只有两个画架，音体美教学设备匮乏，达不到标准要求；计算机室2个、电子备课室1个、电子白板60个、电脑199台，计算机师机比和生机比分别为1：0.7和1：0.07不达标。有藏书室、图书室、阅览室，生均图书25册以上，适合学生阅读，每年按需新增图书，符合标准规定。

3.教师队伍

我校有教职工262人，其中专任教师有257人，专任教师占教职工总数的98%。学历达标率99.6%，其中研究生学历2人，本科学历169人，我校校长岗位培训合格，有培训证，合格证，校长岗位培训率为100%。教职工配备数量适宜，学科结构合理。

1.校舍建设

学校选址和布局基本合理，有土地使用证。校舍建筑总面积20661平方米，生均7.39平方米，每班有标准教室，每个教室63平方米，教学辅助用房实验室、音体美、计算机室等9室齐全，校舍建筑质量符合建筑标准，抗震设防分类标准和防火设计规范。有独立的教学办公室、行政办公室、卫生保健室、总务后勤室等配套办公用房。消防设施能正常使用，每学期定期进行安全隐患排查，并建立安全台账。学生宿舍男女生分别为三层和四层公寓楼，建筑面积6020平方米，一人一床，宿舍配有男女管理员各一人，负责学生的生活和安全，有检查、有记录。按照每40名住宿生配一名生活管理员的标准，还缺6名管理员，未达到标准。学校有食堂，总面积620平方米，就餐人数382人，生均1.89平方米。水冲式厕所38个，旱厕4个，252个蹲位，每层教学楼均有男女生卫生间，达到标准。

校长具有规定的合格学历和职称，经过多次校长任职培训和提高培训，持有“任职资格证书”和“提高培训合格证书”，学校严格实施教师资格准入和持证上岗制度，按照规定督促教师参加继续教育及其他在岗学习培训，培训经费支出高于公用经费总额的5%，达到标准。

3.相关人员配备。

学校有卫生室、心理咨询室，配有专职卫生专业技术员2名，兼职心理健康教师2名，但设施配备达不到《xx省中小学心理咨询室建设标准》要求。专职管理人员10名，低于教职工总数的10%；工勤人员8人，低于教职工总数的5%。达到标准。

4.质量与管理
