通用请假条格式

请假条一般应包括以下几个部分：正文的第一行最中间应写上“请假条”字样。

【格式】

1.居中写标题“请假条”。

2.请假对象的称呼。要顶格写，并在后面加冒号。

3.请假原由。要另起一行，空两格。主要写明请假的原因和请多长时间假。

4.请假起止时间。

5.祝颂语。

春节回家请假条
