七、活动具体安排：

l每天上午10：00~10：30分（周一至周六）为完全英语时间，在班上的所有同学及教师在此时间段内强制使用英语进行交流，可以干其它事，但是，在这段时间里，即使你在和朋友聊天，也必须用英文交流。

l不定期的在完全英语时间里搞一些专题交谈、专题讨论以、专题演讲以及小话剧。在这段时间里，不得干其它事，集中精力搞好我们的每一个活动。

l在我们班群里聊天，必须用英文。

l周六晚英文大片欣赏时间，此时间段内建议大家不要做其它事情，仔细欣赏，看完电影后，每个人都用英语作一个简短的发言（可以是你对该部电影的一些见解，可以是电影中一句让你印象深刻的话。周六晚所有的对话强制使用英文。

l周六电影推荐活动，同学们可以为周六晚的电影选片，但选片的要求是电影是英文的，而且要有英文字幕（不能是中文字幕），同时，推荐电影的同学还要准备好在电影开始之前，对这部电影作一个简单的介绍（英文）。

一、工作范围

二、工作内容

1.总体布局、生产工艺和设备布局、建筑卫生学要求、职业病危害因素和危害程度及对劳动者健康的影响、职业病危害防护设施及效果、辅助用室基本卫生要求、应急救援、个人使用的职业病防护用品、职业卫生管理等。

3.依据国家相关技术规范和标准的要求，通过现场检测和实验室分析，对拟评价项目作业场所职业病危害因素的浓度或强度以及职业病危害防护设施的防护效果进行评定。

三、进度要求

乙方应于合同签订后，在____年____月_____日前完成______________项目职业病危害现状评价报告书的编制工作。

四、文件份数

乙方应向甲方提供职业病危害现状评价报告书8份。

五、双方义务与责任

1.甲方的义务与责任：

1.1 甲方应提供与评价项目相关的下列资料，并对所提供资料的可靠性和准确性负责：

1.1.1 建设项目总平面布置图、车间平面布置图。

1.1.2建设项目工艺流程简介和工艺流程图。

1.1.3原料辅料使用情况(组成、用量等)，生产工艺流程中存在化学物料名称和接触情况。

1.1.4建设项目设备情况。

1.1.5 建设项目职业卫生防护设施：如通风除尘排毒系统设计性能、车间通风换气(全面通风换气量、空气调节及洁净区参数)、事故应急求援设施(紧急救援站的面积、冲洗装置等)、污水处理系统等。

1.1.6 职业卫生管理措施：生产制度、劳动定员、安全操作规程、应急救援预案、作业工人的个人防护、年度职业病防治计划、培训制度、健康监护制度、有害因素监测制度等。

1.1.7 卫生辅助设施：工作场所办公室、更衣室、盥洗室(盥洗水龙头个数)、浴室(淋浴器的个数)、宿舍、食堂的设置情况。

1.1.8其他资料：该项目职业卫生监测档案，职工健康监护档案等。

1.2 根据本合同规定向乙方支付评价费用。

1.3 为乙方到现场工作的人员提供相应的必要的个人防护装备。

2.乙方的义务与责任：

2.1 乙方按第三条规定按时向甲方提交评价报告。

2.2 乙方对评价报告文件质量负责。

2.3 乙方对甲方提供的技术资料负责保密，未经甲方许可不得向第三方泄漏。

六、评价费用和支付方式

根据《 省卫生防疫收费标准》中“劳动卫生学评价”有关规定，经甲乙双方友好协商确定合同总价款为人民币：_______万元整(_______元)，合同生效7日内，甲方向乙方支付合同款的50%，即人民币：______元整 (_____元)，作为前期工作费用。在乙方交付职业病危害现状评价报告，并通过甲方验收后，甲方一次付清合同余款，即人民币：____元整 (______元)。

我的主要爱好是爱看书。说起看书，我还有一段小故事呢!小学5年纪的时候，我看当时的同桌买了一本《茶花女》，这本书我向往了许久，可是爸爸始终没有答应我的要求。我自尊心很强，从小到大几乎没有求过别人，这次，我硬着头皮，只好向他去借。谁知，他说可以是可以，可得拿我的《木马屠城记》跟他交换，我只好把书给他，向他要了那本《茶花女》。

我的缺点就是爱掉哭。呵，你可别笑，这也是我的爱好哩!宠物死了，哭!受欺负了，哭!考试不好，哭!看书看到感人处，鼻子一酸，又落下两排“金豆豆”。不过，我不觉得难为情。我很喜欢一句话“当笑则笑，当哭则哭，无须掩饰。”坦荡荡的。

要说我讨厌的，就数运动了。所以体育成绩也一直不好了。垒球最远能扔四五米，考试成绩从没有过70分的。这个缺点，我以后一定要改。

为保证蓝图公司翼钢电站安全运行，保障在生产劳动过程中人员、设备的安全，参照国家电力行业有关规定，甲乙双方就共同做好电站安全管理工作达成如下协议，共同遵守。

一、甲乙双方必须严格执行《安全生产法》、《电站安全管理规定》、国务院493号令《生产事故报告和调查处理条例》等法规及本协议，承担各自的责任和义务。

二、安全生产、人人有责。甲乙双方应不断强化以安全第一责任人为核心的安全生产责任制。努力改造劳动条件，消除安全隐患，保证工作人员、设备安全。

三、甲、乙双方必须贯彻“管生产必须管安全”到原则，做到在计划、布置、检查、总结、考核生产工作的同时，计划、布置、检查、总结、考核安全工作。

四、甲方责任：

1、电站设备设施及场所应满足国家安全生产法规、标准及规定。因工作需要乙方提出书面计划，甲方为乙方提供安全生产所必须的符合国家标准和规定的安全工器具。

2、督促乙方定期组织工作人员体检并建档。体检不合格或有职业禁忌症者，老、弱、病、残者及未成年工严禁使用。

3、督促乙方做好工作人员的安全教育工作，每年进行一次安全规程、运行规程的考试，建立安全教育、考试档案。

4、督促乙方严格执行“两票三制”工作。

5、审查乙方提交的现场安全运行防护措施。

6、督促乙方对特种作业人员的安全技术培训以及持证上岗。

7、发现乙方违章作业，有权纠正或立即停止其工作。对乙方或乙方工作人员违反有关安全规定、制度，有权按甲方的有关处罚规定予以处理。

8、督促或组织乙方开展安全检查活动和定期召开安全例会，及时向乙方传达甲方有关安全工作的文件、通报和会议精神。

9、组织或参与乙方事故的调查处理。必要时，应聘请第三方专家进行事故调查分析。

10、甲方的人员(包括甲方外请、外聘的工作人员)到电站工作也应遵守电站的现场规程制度，并接受乙方工作人员的安全劝解、监管。

五、乙方职责：

1、遵守甲方颁布的安全、文明生产的管理规定和制度，服从监督和指导。

2、建立健全电站生产运行管理的现场规章制度和安全生产责任制并认真落实。

3、乙方长期从事电站的运行管理，应完全了解电站生产场所和工作岗位存在的危险因素，因此，乙方派驻电站的工作人员，必须对其进行安全教育，告知存在的危险因素、防范措施并严格格执行，乙方须对该电站运行人员进行定期体检，进行安规和安全技术的培训考试，人员名单、考试成绩汇总表报甲方备案;每年3月前向甲方报送工作人员安规、运规考试成绩汇总表，且人员不应随时更换，不得冒名顶替，但不合格人员应立即予以更换。对进入生产现场的甲方及外来工作人员，乙方应按照现场规程制度的要求进行安全劝解、监管。

4、乙方特种作业人员必须持证上岗，每年向甲方报送特种作业证的年审记录。

5、按规定对安全工器具进行定期检查、检验、确保符合安全规定并在检定周期内使用，并向甲方报送检验记录，必须按规定为工作人员配备劳动保护用具、用具。

6、重大施工或技改的施工现场运行安全措施、重大隐患的事故防范措施报甲方审查。

7、乙方必须设立专职或兼职安全员并书面通报甲方，安全员应选择责任心强、坚持原则，熟悉水电站生产和安全有关规程、制度、标准的人员担任，乙方安全员按要求参加甲方组织的安全活动和完成甲方要求的安全工作。

8、乙方应严格执行“两票三制”制度，每月对“两票三制”进行统计、分析，做出合格率评价，报甲方审查，乙方按时向甲方报送事故卡片、事故统计报表、年、月报表以及每月发生的不安全情况。

9、对甲方提出的监察性意见，必须按要求组织整改并及时回复。发生事故必须及时报告甲方，并采取应急救援措施。按“四不放过”的原则调查处理事故，并向甲方报送事故分析处理报告。

六、其他：

1、甲、乙方按事故调查报告结论承担相应的经济和考核责任。

2、甲方按照规定对乙方或乙方个人的违章处罚及其他安全方面的处罚可在每月支付给乙方的运行、维护、生产管理费用中扣除。

3、乙方不服从甲方管理或严重违章作业、管理混乱、事故不断，甲方有权终止合同。

4、未尽事宜，经双方协调解决。

七、本协议自甲乙双方签章后生效。
