一、安全生产目标

伤亡控制指标：死亡事故为零，重伤事故为零，轻伤事故控制在1.2%以下，火灾事故为零，。

1. 安全文明施工管理目标：无安全事故

2. 甲乙双方必须认真贯彻执行国家及建设部办法的有关安全生产法律法规、规范、标准、规程及省建设厅、市建委、市施工安全监督站颁发的有关安全生产、文明施工、消防安全、治安综合治理的有关文件规定和要求，严格遵守安全生产管理制度，落实安全文明施工管理目标。

二、甲方责任

1. 审核乙方的从业资格和安全生产资格，是否符合本工程建筑机械施工的要求

2. 将现场的安全管理制度、安全管理目标、安全生产概况和要点向乙方进行交底，对乙方施工全程进行监督。

3. 甲方和乙方安全负责人，应统一协调工程起重机械施工现场的安全管理工作。

4. 提供本工程目前安全生产的动态，提供相关的安全宣传资料，指导乙方有效地开展安全生产工作。

5. 指导乙方对员工进行安全教育和协助乙方的安全管理，提高乙方施工人员的安全意识和技能。

6. 指导、协助乙方按上级主管部门有关规定，做好安全管理资料的收集和整理工作。

7. 对乙方施工过程中存在的安全隐患，应及时指出并责令整改，整改合格后才能进行施工，如乙方拒不整改，甲方必须采取强硬措施杜绝安全事故的发生。

8. 如发生重大上伤亡事故，必须派人指导，协助乙方做好事故的善后处理工作和救援工作。

9. 根据《建筑工程安全生产措施费管理方法》，控制好本工程安全生产设施费，按进度分阶段验收乙方的安全措施，经安监机构同意、监理通知，收到业主付款和甲方审核安全生产措施费落实情况后，支付乙方安全措施费，支付比例按《建筑工程安全生产措施费管理方法》执行。

三、乙方责任

1. 遵守国家、省、市、建筑公司所颁发的各项安全法规、行业标准和甲方的安全管理制度(程序、应急预案)，建立和健全与现场安全管理相适应的安全管理制度和安全生产责任制及操作规程。

2. 应按有关规定保证安全生产措施费专款专用，遵守有关工伤保险的缴费规定，为所有施工人员办理工伤保险，降低生产安全风险。

3. 严格按照劳动保护法，定期发放工资。

4. 进入施工现场后，服从甲方的管理，落实施工人员的安全生产责任，指定安全生产责任人，成立安全生产架构，配备专职或兼职安全管理人员，开工前把指定的安全生产责任人和安全管理人员名单报甲方项目部。落实安全管理职责，指定安全管理措施，不违章指挥，不违章作业，积极开展安全生产教育培训工作，不断提高员工的安全意识和安全生产技能，保证员工持证上岗，减低安全事故的发生几率。

5. 开工前将指定的各项安全生产制度、方案、措施报甲方和监理按相关程序审批，审批后严格按制定的安全生产制度、方案、措施施工。

6. 进场前将施工中需使用的设备、设施书面报甲方项目管理部，同时遵守设备设施的安全施工规定。

7. 施工中需要搭设、安装安全设施、临时用电、使用设备、施工方案的编制、材料的检验、装拆资质、施工人员的从业资格、完工后的检验、检测等，必须符合国家、行业标准和安全管理要求。

8. 对施工中可能出现的危险或危害，应及时采取防范措施，同时向项目部报告，由项目部通知其他施工单位，共同防范。

9. 必须遵守《建筑施工安全检查标准》和当地建设工程质量安全监督站的管理规定，填写好安全运行记录，定期上交给甲方。

10. 做好现场防火工作，必须贯彻“预防为主，防消结合”的方针，立足于自防自教，自觉遵守《施工现场防火规定》。

11. 发生安全事故时，必须严格遵守中华人民共和国国务院第493号令《安全生产事故调查报告和处理条例》的规定编写事故报告和调查处理。

四、其他约定

1. 乙方在施工时间，必须接受甲方的安全检查，没有达到甲方的安全生产管理要求，每次罚款300元，安全生产责任人不参加检查的每次罚款300元。

2. 乙方不及时编写方案、安全运行记录，每次扣款300元。

3. 乙方必须建立安全生产措施费台帐，填写好安全生产措施费报表，安全生产措施费必须专款专用，甲方对安全生产措施费的使用进行监督，没有建立安全生产措施费台帐或将安全生产措施费挪作他用的，从重处罚。

4. 乙方被国家质量安全管理机构通报，提出停工整顿，造成甲方停工扣分时，在工程款中扣除所有的安全生产措施费。

5. 甲方项目安全员有对乙方安全生产进行监督的权利，对安全措施得不到落实，安全隐患较多，安全隐患得不到及时排除的情况，有进行限期整改和罚款的权利。

6. 乙方必须服从甲方《安全生产奖罚条例》的规定

7. 甲方统一购买安全生产管理资料，施工质量管理资料，安全设备材料，安全管理教育培训资料等发放给乙方(成本费由乙方支付)。

8. 乙方未按施工所在地区建设主管部门要求办理平安卡的，每人罚款50元。

9. 甲方发现乙方的安全员、电工、焊工、装拆工未持证上岗，每人罚款50元。

10. 乙方的施工人员必须参加岗前安全知识考核，未参加人员每人罚款50元。

11. 乙方施工人员进入施工现场必须按规定戴好安全帽，管理人员戴白色安全帽，专兼职安全员戴红色安全帽，生产工人戴黄色安全帽，高空作业必须系好安全带。

为确保______电气化铁路线路区段内捆绑、加固作业安全及人身安全，明确双方的安全责任、权利和义务，经双方共同协商，特制定如下补充协议。

第一条 甲乙双方基本情况

第二条 甲方人员及劳动防护用品设置

1.驻站联络员：__________名。

2.安全防护员：__________名。

3.安全管理员（由作业队队长兼任）：__________名。

4.对讲机：__________台，驻站联络员、安全防护员、安全管理员各__________台。

5.绝缘用品：参与作业人员必须配备绝缘靴、绝缘服及绝缘帽。

第三条 甲方安全责任

1.甲方管理人员根据发运货物的需要，有权向乙方下达捆绑加固服务任务，并进行监督。

2.甲方驻站联络员要严格执行《呼和浩特铁路局电气化铁路区段货装作业安全办法（试行）》有关规定，与车站联系确认作业时间、股道后，方可开始作业。

3.甲方管理人员必须在距车列两端20米处设置符合规定标准的安全警示标志，必须执行先防护、后作业的要求，装卸作业必须在安全警示标志之间的区段内进行。

4.乙方作业人员到达作业场地，开始作业前，甲方驻站联络人员和安全防护员员必须确认作业股道正确、作业车辆在安全作业标志内侧，方可开始作业。作业结束，必须确认所有人员、工具及装卸设备均已离开危险区域后，防护信号已撤除后，方可通知车站作业完毕。

5.甲方管理人员如遇雨、雪、雾天气必须进行接触网停电装卸作业，发现接触网设备异常，应立即停止装卸作业，并及时上报。

6.甲方有对乙方进行铁路安全常识宣传的义务。

7.对乙方现场作业中涉及安全、服务质量、装卸管理的各类违反规章、规定的问题，有制止、指挥的权利。

第四条 乙方安全责任

1.乙方作业人员要严格按照《铁路装卸作业安全技术管理规则》、《铁路装卸作业标准》等铁路规章制度和本协议确定的作业地点、范围进行捆绑、加固作业，确保铁路行车安全和作业安全。

2.乙方作业人员严禁未经电气化安全培训或考试不合格人员进入电气化站场作业。

3.乙方作业人员严禁作业人员未得到通知，擅自进入站场。

4.乙方作业人员严禁私自带长大工具进入站场，捆绑加固、调整车门等作业工具，必须经装卸负责人审核后方可使用。

5.严禁乙方作业人员越区作业，必须在指定区域内作业，不得在站场内随意走动。

6.严禁乙方作业人员在车列两端未采取安全防护措施而进行作业。

7.乙方作业人员在带电的接触网下，严禁爬上车辆顶部作业，严禁向上投抛篷布绳索、绳网，严禁用竹杆等物测量货物装载高度、宽度，严禁用水流冲刷车辆。

8.乙方装卸工组在通过电气化铁路道口或进入带电接触网区域时，所携带的长型工具（如铁锹、撬棍、推拉杆等）一律保持水平状态；起重机臂杆、装载机铲斗、叉车门架等距接触网均不得小于2m，驾驶室外禁止坐人或站人。

9.在接触网不停电的条件下进行装卸作业时，乙方作业人员、工具及装卸设备与接触网的带电部分需保持2m以上的距离，并派人监护，必要时应在车上安装安全防护网。

第五条 事故应急处置措施

1.一旦发生人身伤亡事故，现场作业人员应立即报告现场负责人，现场负责人必须立即判断事故情况，迅速采取有效措施，组织抢救，防止事故扩大。遇有人员伤亡时，以及时救治为原则，实施对受伤人员的应急救治，立即通知120急救中心救护。同时，向公司安监部门和公司领导汇报。

2.人员发生触电立即将触电者脱离电源。

（1）关闭电源开关。一是立即通知车站断电；二是带上绝缘手套，穿上绝缘鞋，用相应电压等级的绝缘工具按顺序拉开开关；三是抛掷裸金属线使线路短路接地，迫使保护装置动作，断开电源。

（2）低压触电时，可采取切断电线、挑开电源线、拉开触电者等方法，使触电者脱离电源。

（3）对触电者现场进行紧急心肺复苏救治并及时送至医院进行抢救。

第六条 违约责任

1.由于乙方责任造成铁路行车或货运事故、路风事件等有损铁路和甲方声誉问题以及有违反法律法规行为时，甲方有权解除本协议并扣除向乙方支付的捆绑、加固作业费，情节严重的，移交铁路公安部门进行处理。

2.捆绑、加固作业中途乙方有故意捣乱和违反本协议规定事项的，甲方有权扣除乙方劳动产生装卸费总额的50%；有中途弃岗的人员，甲方有权扣除该人员本次作业所得劳动报酬。

3.在现场酒后作业，甲方有权终止乙方作业。

4.因乙方违章作业、不按规定操作等原因造成人员受伤，甲方不予支付其受伤产生的任何费用。

第七条 本协议执行过程中，如发生争议，由双方协商、协调解决；若经协商、调解不能解决争议的，任何一方可以向当地人民法院提起诉讼。

第八条 本协议经双方签字盖章后生效。

第九条 本协议一式四份，甲乙双方各执两份。

能当上__百年校庆的志愿者是多么荣幸的一件事!在我得知自我被校团委青年志愿者服务中心录取为校庆交通指挥组的志愿者时，心里还是很兴奋的。能在自我的人生当中遇上这么一件喜庆的事，实属难得!

在这次的志愿服务过程中，我获益良多。先不说丰盛的三餐，__为我们精心安排以外，在这次的活动中，我更是收获了友谊和感动。从__校庆的彩排过程中，我就结交了来自不一样学院，但都热心为校庆服务的志愿者们。从组长们的合理分工，组员之间的相互配合，我更是体会到一个团队的力量。到了正式上岗的那一天，天还没亮就得从床上爬起来，为的只是为校庆出一份微弱的力量。从跃进北步行来到保卫处，看到大家都穿上志愿者的马甲，戴上专用的袖章，我们按照原计划站在指定的岗位上，大家都喜上眉梢。刚开始车流量还不太大，我们尚算应付得来。可是随着时间逐渐地推移，人流量、车流量逐渐增多，我们看到穿着色彩斑斓的校庆志愿服，大家都昂首阔步地向会场迈进。车辆开始陆陆续续地驶入我们负责的3号停车场，我们就按照间隔好的位置指引车辆停好。司机们都很配合，我们也干得很投入。之后我负责指挥的那部车停好了，从车里走出一位高大的外国人，他向我友好地微笑，还向我问__运动场要怎样走，他并没有用英文来问我，而是用了娴熟的普通话。可是，我最终发挥了自我英语专业的优势，用一口流利的英文解答了他的问题。显然，他满意地笑了，我也感到内心的愉悦。

到了11点左右，本来按原本计划是吃饭时间，可是车流量到达了高峰期。每当有车辆驶入，我们都必须跟着车辆跑到停车线，如此一来，我们跑了不明白有几十趟了，并且还要跑到上头的那一层去，基本上是没得休息了。之后，车位满了，我们还需要指引司机走到六一区去停车。向我们问路的校友更是数都数不清，但我都耐心地向他们解答了问题，为校庆出一分力。

早上的志愿服务历时4个多小时才结束，毋庸置疑，这次的志愿服务很考验我的耐力和持久力，看着同伴们这么辛勤地工作，我也不忍心去休息。我想，在她们身上，我真的学到了很多，包括她们坚持不懈的忍耐力和乐于助人的精神。

晚上的站岗时间不是很长，到了休息时候了，我也只能说句不舍，我想再辛苦也是值得的。毕竟华农的百年校庆，就仅有这么一次。能在这次校庆活动中展现当代志愿者的风采，是我们的骄傲。到此刻，每每看到我们交通指挥组的合照，我都十分感动，那是珍贵的友谊，那是一笔宝贵的财富，让我受益无穷。
