二、甲方的义务、权利

(4)协助乙方配备安全防护用品和安全生产学习材料。

(5)发生安全事故时，协助乙方报警，协助救护，协助进行事故调查。

2、甲方权力

(1)甲方有权检查乙方的安全生产情况，发现安全问题、安全违章因素时，有权提出改进、改正意见。出现严重违规行为，经教育不改时，甲方有权终止相关协议，劝其退出作业区。

(2)甲方有权要求乙方参加甲方的安全活动或安全会议。

(3)甲方进行安全生产检查，对违反安全生产规章制度的乙方人员有权进行现场处罚。

三、乙方的义务、权力

1、乙方义务

(1)必须认真贯彻落实《安全生产法》、《道路交通安全法》及相关安全政策、规定和制度。

(2)乙方进入施工现场必须参加甲方组织的安全技术交底，了解操作规程，熟知安全要求，严格执行安全操作规定，安全生产、安全施工。

(3)积极参加甲方组织召开的安全例会及各项安全活动，支持甲方组织的安全检查，认真整改甲方提出的安全隐患及所存在的问题。

(4)乙方进入工地的人员、设备，必须符合安全生产的条件，配齐所需的各类安全防护用品，特别是反光衣、号帽、安全警示牌。对铝合金跟踪杆或标尺进行绝缘处理。

(5)及时对测区进行踏勘、巡视，明确危险源(点)，提前制定预防措施。

(6)发现不安全因素时，除积极主动消除不安全隐患外，应及时报告给甲方。发生安全事故时，在积极救治、处理的同时，应及时通知甲方。

(7)发现不按规定作业或发现违章、违规现象时，应及时制止，消除隐患。

2、乙方权力

(1)对甲方制定的各项安全生产规章制度有异议时，有权力以书面形式通报甲方。

(2)认为测区存在着严重不安全隐患，且无法防护时，乙方可以要求重新安排新测区。

(3)施工作业过程中，发生“安全”与“生产”相矛盾时，以“安全”作为第一原则，可以停止生产，并报甲方协助解决。

(4)在施工过程中发现甲方的违章指挥，乙方有权拒绝接受，并及时向甲方的上级领导反映。

四、安全责任及安全经济责任

1.由乙方违章指挥、违章作业、违反劳动纪律而造成的伤、残、亡事故，一切经济责任由乙方负责。

2.由甲方责任造成乙方施工人员伤、残、亡的，由甲方按国家现行规定给予乙方一次性经济补偿。

3.由甲、乙双方责任造成的事故，双方协商解决;发生异议时按国家规定，由上级主管部门裁定后处理。

五、处罚制度

乙方有下列情况之一，甲方有权给予100-500元的罚款，或者令其停工检查;情节特别恶劣的，甲方有权终止协议，由此造成的经济损失由乙方承担：

1.严重违章，直接危及人员生命安全的，而又不认真接受整改意见的;

2.同类违章行为多次发生的;

3.无视安全生产，不按安全施工要求及安全操作规程作业的;

4.违反劳动纪律、违章作业造成事故的;

六、其他

1.本协议未尽事宜，由甲、乙双方协商解决。

一、上学期本学科基本情况和分析

每次的上课中都是我怕收获的过程，每次老师都会给我很多很好的建议，经过时间的洗礼和经验的增加，我逐渐变得成熟了，我在进步的路上，因为我一直在努力。

三、班主任工作总结。

首先，必须要树立良好的班风和学风。

作为一个班级，一个集体，没有良好的班风和学风是打造不出一个好的班级的。我代理的是初三五班的实习班主任，五班在王老师的带领下已经是综合班级量化第一，我要做的就是将这种好的氛围继续保持下去。加强对学生的管理，增强班级凝聚力。

其次，学生班干部提高班级管理能力。班级本来就是学生是一份子，我一直认为学生应该参与到班级管理之中而不是强调班主任的管理，五班的班长姜耀爽同学担任校学生会主席，有很强的班级管理能力，五班的班干部们也都在努力。他们成为班主任管理班级的强有力的助手。

再次、加强沟通和交流。作为班主任，要经常和学生沟通，了解学生的心理发展和学习情况，每个小周的周二下午是班会，都会找几个同学来谈谈他们最近几天发生的高兴或者不高兴的事情，这都有助于班主任及时了解学生情况。

下面我描述一下我在小学期间的主要工作内容：

第一，作为大队委，协助大队辅导员做学校宣传报，广播活动，升旗仪式;大队老师不在时主持大队会议，并分配大队委员的工作任务，积极检查执行情况;负责总结大队的工作执行情况，并记录在日志上;第二，作为班主任的小帮手，4年级开始早去学校10-20分钟，在早自习时为同学出两道疑难题或典型例题，帮着提高班级成绩;帮着老师改判作业，统计分数，使用家校互动平台通知家长;班级同学有不会的问题或者事情能够积极帮着解决;协助老师维持课堂纪律;我所做的手抄报经常被作为优秀作品表扬或者在学校展示;我的幻灯片制作不仅是班级小组时拿第一，还经常应用在大队工作中;团队意识强，经常带领小组成员积极分配，完成小组任务，并获第一;

第三，4年级进入国旗班，当过升旗手，领操员，独立能力强，善于思考，善于沟通，责任心强，比如放学时看到国旗没有收起来会帮着收回来;比如最后离开教室时看看教室的窗户是否关好，风扇，灯开关是否关好;上课了，黑板没有擦好，会督促值日生做或者自己去做;我见黑板布太破了，我从家里拿来一块新抹布等等，我认为责任心体现在为班级，同学，学校所做的一点点细节上。

我申请参与竞选班长职务。若班里有比我更适合的同学当班长，我也一定会支持，并希望担任学习委员或科技委员，或团支书中的职务。因为我责任心强，有积极向上的精神，有精力，学习努力，助人为乐，希望自己的奉献精神得到认可。
