为确保公司的长远可持续发展，为了独立核算部合理收益，为了扶优汰劣强化管理，在充分考虑责、权、利统一的基础上，经甲乙双方充分协商，签订如下业务承包独立核算责任协议，共同遵守:

(一)乙方向甲方上缴费用比例情况一览表：

可以减少上缴费用比例的几种条件：

1、乙方每有一名注册监理师在甲方注册，管理费用减少一个百分点;乙方每有一名注册造价师或者注册一级建造师，管理费用减少半个百分点;

2、乙方上年度(1月1日到12月31日期间)的监理费回款数额大于等于__________________万元，管理费用可减少一个百分点;

3、乙方若为甲方的股东，管理费用可减少两个百分点;

4、乙方年底前可向甲方账户缴纳质量安全保证金，每缴纳__________________万元，管理费可减少一个百分点。(注：工程竣工无重大质量安全事故，甲方将保证金退回乙方，无息)。

特殊情况，签署监理合同前，除必须上交的公司管理费外，乙方必须另外提前缴纳的额外管理费的两种条件：

1、 监理合同实际监理费(此数值为B)收取比例低于国家670号文百分之五十(此数值为A)的，经甲方管理层合同评审原则上不能签署，如果乙方有足够的理由，经甲方同意则可以签署，但是需在合同签署前向甲方一次性缴纳额外管理费[(A-B)*2%]。

2、 监理合同监理费数额在5万以下单项合同，经甲方管理层合同评审原则上不能签署，如果乙方有足够的理由，经甲方同意则可以签署，但是需在合同签署前向甲方一次性缴纳额外管理费__________________元。

(二)监理费到账后的分配管理：

1、 独立核算部门的监理费由乙方负责向业主催收，若乙方在甲方开具监理费发票，那么乙方必须将与开具监理费发票票面相符的全额监理费打入甲方基本户，若乙方在工程所在地属地开具监理费发票且在属地纳税，则乙方也必须将全额监理费打入甲方基本户,具体执行由甲方财务部门统一管理拨付。

2、甲方要求乙方采用“乙方向甲方上缴费用比例情况一览表”中第一种方式与甲方结算，即乙方要负责监理费收入与费用支出在甲方财务的平帐工作，乙方须提供人力成本及入账费用的票据并及时平帐，人员工资成本平摊最多只能占到账监理费的70%，其余30%均须采用可入账的费用票据进行平摊，甲方财务部门将根据乙方的平账情况在乙方所到账监理费范围内拨付与乙方所平账目相等的费用，乙方须在项目进行期间平时及时向甲方财务部门上报人员身份证明及每月考勤表和相对应的人员月工资表，乙方还须按月及时将入账的费用票据及时报与甲方财务部。

3. “乙方向甲方上缴费用比例情况一览表”中第二种方式只有在极特殊的情况下由乙方负责人书面陈述理由并提出临时采用第二种的申请，该申请只有在甲方召开评审会议且会议最终同意后方可采用。

4、若乙方在工程所在地属地税务部门代开监理费发票且在属地完税后，乙方须持完税税票在财务部门报销，等与税票相对应的监理费到账后甲方对报销的税票予以拨付，报销完税票后的其余做法同第一种方式。

5、乙方的人员成本列支及费用票据入账须符合国家税务部门及甲方的要求，此条解释权归甲方财务部。

(三)招投标及人员证书的使用管理：

乙方的招投标活动应自主完成，投标报名前须向甲方交纳__________________元投标保证金，如果工程中标则保证金无息退还乙方，如果未中标则作为陪标费收归甲方;如果乙方需要甲方找陪标单位，则每找一家乙方需向甲方支付__________________元陪标联系费。如果乙方需要甲方代做标书，则每做一家收取__________________元标书费(包括装订)，如果乙方需要甲方人员参与投标活动，则乙方向甲方支付每人每天不少于__________________元劳务费。 乙方人员的国家注册证书、职称证等必须交由公司统一管理，考虑到资源有限和机会成本，甲方鼓励乙方组建自身具备任职资格的监理人员队伍并使用自身人员进行生产经营活动，若因业务需要而申请由甲方提供人员证书进行相关活动，在证书有偿使用的原则下，乙方均须向甲方缴纳甲方规定的甲方人员证书使用费。

1、关于合同、质监及安监等备案：乙方若利用甲方培训或注册的人员证书进行备案，则乙方须向甲方缴纳证书使用费，具体为：缴纳使用费的时间区间为自备案记载日起至工程竣工备案完成日止，国家证(包含注册监理师和注册造价师)每月__________________元/个，其他证书每月__________________元/个，此使用费随每笔监理费到账后按已发生的使用时间即时扣除。

2、关于招投标、工地检查及其他与公司经营业务有关的用证情况：

⑴营业执照、资质证书、组织机构代码证、投标企业信用手册、税务登记证借出时，自借出之日算起至还证之日止，当天归还免费，超过一天的按每证每天__________________元收取。

⑵国家证(包含注册监理师和注册造价师)自借出之日算起至还证之日止，当天归还免费，超过一天的按每证每天__________________元，其他证书(如省监理证、见证员证、职称证)当天归还免费，超过一天的按每证每天10元，此使用费随每笔监理费到账后按已发生的使用时间即时扣除。

(四)双方权利与义务：

1. 乙方负责本部门的一切监理工作，如负责投标前后的经营管理、办理备案手续、监理项目的竣工验收备案等全部监理工作并承担与此活动有关的一切费用。

2. 乙方负责招聘参与本部门的所有监理人员，招聘条件参照甲方制订的统一标准，并且在上岗前将每人的身份证、学历证、职称证的复印件报甲方备案。乙方负责本部门人员的工资福利的标准确定和发放。乙方负责现场监理人员的安全教育和管理责任并承担相关费用。

3. 乙方负责管理其项目工程，其工程备案的总监理工程师原则上要求必须由乙方符合任职资格的人员担任(乙方须将其相关执业资格注册在甲方单位)。

4、若因迎检等需要甲方领导、甲方备案总监或备案监理人员出席与监理相关的活动，则须由乙方支付劳务报酬(甲方领导每天不少于__________________元，总监不少于每天__________________元，其他监理人员不少于每天__________________元)和出席活动的其他成本支出。

4. 乙方负责其管理的工程项目的对内对外的协调，并承担由此活动产生的一切费用，若乙方在协调时需动用甲方资源，乙方还需酌情支付给甲方资源占用等费用。

5. 甲方宏观负责本工程的监理运作，若因监理工作不力给甲方造成损失或不利影响，甲方有权单方面处罚乙方;若监理工作出色给甲方赢得锦旗或者受到政府主管部分的嘉奖，甲方酌情给予奖励。

(五)甲方管理和指导

1.乙方要严格遵守法律法规并遵守甲方的规章制度和奖罚制度，乙方要积极配合甲方的现场检查和指导。

2.乙方按甲方要求报交甲方所需的监理资料：如监理规划、监理月报、监理工作总结、现场监理人员考勤表等资料。

3.甲方应定期或不定期对乙方工程项目进行检查巡视并进行管理指导。

(六)本协议为标准文本，无特殊情况不能更改条款。本协议有效期原则上不超过一年，每年12月底续签下一年度协议。本协议一式叁份，甲方贰份乙方壹份;具备共同法律效力。

(七)本协议自双方签字的年度月份开始执行，_________年_________ 月_________ 日至_________年_________月_________日。

一个老师告诉我：备课充分，能调动学生的积极性，上课效果就好。但同时又要有驾驭课堂的能力，因为学生在课堂上的一举一动都会直接影响课堂教学。因此上课一定要设法令学生投入，不让其分心，这就很讲究方法了。上课内容丰富，现实;教态自然，讲课生动，难易适中照顾全部，就自然能够吸引住学生。所以，老师每天都要有充足的精神，让学生感受到一种自然气氛。英语是一门外语，对学生而言，既生疏又困难，在这样一种大环境之下，要教好英语，就要让学生喜爱英语，让他们对英语产生兴趣。否则学生对这门学科产生畏难情绪，不愿学也无法学下去。

一、课堂教学

英语的作业主要是英语周报以及课课练的试卷。初中的课程很注重练习。每天都必须一定量的练习训练。英语作业大都是选择题，所以改起来不算麻烦。不过还是发现了自己改作业的缺陷，同样一个班的试卷，别人十来分钟就批完了，自己恁是要花上个把个小时。后来还是朱老师教了自己，原来批改作业也有方法讲究的。若将答案写在另一张纸上，abcd答案与试卷答案的间隔一样，批改起来只需将不一致的划掉就可以了，这样批改起来效率果然可以高很多。批改时发现有些题目学生错的比较集中，甚至答案都错到一个选项上，说明那个题目很容易被误导到那个错选项上。这样讲评时便需要着重讲解。批改作业主要也就是为了了解学生的掌握程度，以及查漏补缺吧。

讲课时时间的安排要紧凑，发现自己讲课时说话太慢，讲的内容少而杂乱，没有形成程序。以后还是要注重锻炼自己的速度，思维要敏捷，做事情要干练。

二、班主任工作

带我的班主任工作的指导老师也是我的曾经初中时候的班主任，一向欣赏她工作作风的我有幸跟着她学习。做实习班主任期间，曾老师教会我如何培养班干部的能力以及如何凝聚班集体。我自己也常常观察学生，了解他们的变化并在需要时给予辅导 。

三、对自己英语课堂教学优缺点的总结

结合指导老师、实习老师、同学们的意见以及自己的思考，我对自己此次英语课堂教学优缺点做如下总结

1. 课堂流程整体把握不错，上课有章法，遵循纯阳学校规定的“四步流程教学法”或“PPP”教学法等。

2. 课堂上教态自然，教学耐心;常微笑，常鼓励学生;对学生一视同仁，平等对待。

3. 讲解知识点的同时，能注意学生能力、策略、情感及态度的培养。

4. 板书的设计与书写需要提高;课堂举例要有意义，要举经典例句

5. 讲具体的一节课要有重点，要有简明的课堂小结

6. 声音要再洪亮一些，有升降。

7. 对睡觉、玩手机的学生管理应该严厉一些

8. 在讲解知识点时，要按照考试大纲分轻、重。

四、对英语课堂教学与考试的思考

在听课与讲课的过程中，我听取其他老师的意见，结合自己的心得体会，觉得作为一个好老师，不但要对基本的专业知识有深刻熟练的把握，还要对考纲、考点十分了解，更重要的是弄清楚考试与课堂教学的关系并将其应用于实际教学中。这样在讲课过程中，让学生们打下扎实的英语基础的同时，考试也能考出好成绩。

考试、中考并不与我们在大学里所学的教学法对课堂教学的要求相矛盾。考试是形式，是一个让老师了解自己教学效果的手段，也是一个让学生总结学习方法，促进学生学习的助推器。只是现在好多老师对这一手段、助推器的理解不对，应用的不是很好，所以因此导致了一些问题。现在的考试内容过于机械，频率过高，老师、学生，甚至领导、家长对考试的意义存有不当的理解，以致对英语课堂教学产生不良影响——上课以考试为目的。这恰恰是本末倒置。考试，重知识，轻技能;重语法与词汇，轻情感与策略。从而误导老师上课也如此。所以我认为要做一个合格的老师，应该仔细研究考纲、考点并把它灵活的运用到课堂上，并且善于出考试题，出有质量的考试题，更加善于让考试服务于课堂教学。当然做到这些需要学校领导的认可与支持。这些都是我以后的努力方向。
