二、个别医疗机构在办理《出生医学证明》时未按要求将新生儿和新生儿父、母亲的个人信息存档，《出生医学证明》存根联未按要求粘贴在《出生医学证明首次签发表》的右下方。

三、个别医疗机构未按要求使用统一的《出生医学证明》登记表。

五、保证金Deposit

八、合同终止及解除的规定Terminationanddissolutionofthecontract

九、违约及处理Breachofthecontract
