在科室教师对我们很好，总鼓励我们，要敢做，不能什么都只是看，要自我亲自动手实践才能记忆深刻才能真正掌握。每一天都有教师给我们讲课，从血液透析的原理、方式、如何护理等方面为我们进行详细的讲解。透析的整个流程：每次透析前都要为病人测T、P、BP(体温、脉搏、血压)，还有就是病人的体重。每隔一小时都要监测并记录病人的血压、静脉压、超滤率、TMP值、电导、电流量等值，这些数值看似简单，其中的学问大的很，因为病人在透析时是建立了一个体外血液循环通路，很容易引起高血压和低血压等并发症，插管感染或透析器被污染都会引起感染，伴随的症状就是发热。所以，每次透析我们都要检测这些数值，以防病人发生问题。

除了这些，我们还要为病人做宣教，这一项也是很重要的。做AV瘘的患者，做瘘的一侧胳膊不能测量血压、提重物，刚刚做瘘的病人应自行锻炼，锻炼的方法很简单就是松握拳练习。插管的病人要嘱其不能自行拔管，要常常测量体温，为防止感染，要坚持插管周围的皮肤清洁、干燥。饮食啥工要注意食用优质蛋白、高热量、高磷低钙、低钾低钠的食物。

在血液透析室的一个月，忘不了第一次为病人患要时的紧张，也忘不了熟练了之后的轻车熟路;忘不了第一次为病人注射利比奥时的紧张发抖，也忘不了掌握了注射技巧之后的自信熟练。感激教师的教导，更要感激病人肯给我机会。护士长曾说，不是我们培养出一批批优秀的护士，是病人把他们培养出来的，如果不是病人给他们机会去实践，谁能练出手?我认为护士长的话很有道理，也一向记得。

一个月的实习就要结束了，虽然说血液透析室的专业性很强，但我相信在那里所学到的东西将会在我以后实习的科室受用，在此感激护士长和教师们的教导，我会继续努力，运用所学到的知识，在以后的科室中好好表现。

__年是全国卫生系统继续深化改革的一年，全体护理人员在院长重视及直接领导下，本着“一切以病人为中心，一切为病人”的服务宗旨，较好的完成了院领导布置的各项护理工作，完成了20__年护理计划___%以上，现将工作情景评价

一、认真落实各项规章制度

严格执行规章制度是提高护理质量，确保安全医疗的根本保证。

1、护理部重申了各级护理人员职责，明确了各类岗位职责制和护理工作制度，如职责护士、巡回护士各尽其职，杜绝了病人自换吊瓶，自拔针的不良现象。

2、坚持了查对制度：

(1)要求医嘱班班查对，每周护士长参加总核对1～2次，并有记录;

(2)护理操作时要求三查七对;

(3)坚持填写输液卡，一年来未发生大的护理差错。

3、认真落实骨科护理常规及显微外科护理常规，坚持填写了各种信息数据登记本，配备五种操作处置盘。

4、坚持床头交接班制度及晨间护理，预防了并发症的发生。

二、提高护士长管理水平

1、坚持了护士长手册的记录与考核：要求护士长手册每月5日前交护理部进行考核，并根据护士长订出的适合科室的年计划、季安排、月计划重点进行督促实施，并监测实施效果，要求护士长把每月工作做一评价，以利于评价经验，开展工作。

2、坚持了护士长例会制度：按等级医院要求每周召开护士长例会一次，资料为：安排本周工作重点，评价上周工作中存在的优缺点，并提出相应的整改措施，向各护士长反馈护理质控检查情景，并学习护士长管理相关资料。

3、每月对护理质量进行检查，并及时反馈，不断提高护士长的管理水平。

4、组织护士长外出学习、参观，吸取兄弟单位先进经验，扩大知识面：5月底派三病区护士长参加了国际护理新进展学习班，学习结束后，向全体护士进行了汇报。

三、加强护理人员医德医风建设

1、继续落实护士行为规范，在日常工作中落实护士礼貌用语50句。

2、分别于6月份、11月份组织全体护士参加___宾馆、___宾馆的礼仪培训。

3、继续开展健康教育，对住院病人发放满意度调查表，(定期或不定期测评)满意度调查结果均在95%以上，并对满意度调查中存在的问题提出了整改措施，评选出了最佳护士。

4、每月科室定期召开工休座谈会一次，征求病人意见，对病人提出的要求给予最大程度的满足。

5、对新分配的护士进行岗前职业道德教育、规章制度、护士行为规范教育及护理基础知识、专科知识、护理技术操作考核，合格者给予上岗。

四、提高护理人员业务素质

1、对在职人员进行三基培训，并组织理论考试。

2、与医务科合作，聘请专家授课，讲授骨科、内、外科知识，以提高专业知识。

3、各科室每周晨间提问1～2次，资料为基础理论知识和骨科知识。

4、“三八妇女节”举行了护理技术操作比赛(无菌操作)，并评选出了一等奖、二等奖、三等奖分别给予了奖励。

5、12月初，护理部对全院护士分组进行了护理技术操作考核：病区护士考核：静脉输液、吸氧;急诊室护士考核：心肺复苏、吸氧、洗胃;手术室护士考核：静脉输液、无菌操作。

6、加强了危重病人的护理，坚持了床头交接班制度和晨间护理。

7、坚持了护理业务查房：每月轮流在三个病区进行了护理业务查房，对护理诊断、护理措施进行了探讨，以到达提高业务素质的目的。

8、9月份至11月份对今年进院的___名新护士进行了岗前培训，资料为基础护理与专科护理知识，组织护士长每人讲一课，提高护士长授课本事。

9、全院有___名护士参加护理大专自学考试，有___名护士参加护理大专函授。

五、加强了院内感染管理

1、严格执行了院内管理领导小组制定的消毒隔离制度。

2、每个科室坚持了每月对病区治疗室、换药室的空气培养，对高危科室夏季如手术室、门诊手术室，每月进行二次空气培养，确保了无菌切口无一例感染的好成绩。

3、科室坚持了每月对治疗室、换药室进行紫外线消毒，并记录，每周对紫外线灯管用无水酒精进行除尘处理，并记录，每两月对紫外线强度进行监测。

4、一次性用品使用后各病区、手术室、急诊室均能及时毁形，浸泡，集中处理，并定期检查督促，对各种消毒液浓度定期测试检查坚持晨间护理一床一中一湿扫。

5、各病区治疗室、换药室均能坚持消毒液(1：400消毒灵)拖地每日二次，病房内定期用消毒液拖地(1：400消毒灵)出院病人床单进行了终末消毒(清理床头柜，并用消毒液擦拭)。

6、手术室严格执行了院染管理要求，无菌包内用了化学指示剂。

7、供应室建立了消毒物品监测记录本，对每种消毒物品进行了定期定点监测。

六、护理人员较出色的完成护理工作

1、坚持了以病人为中心，以质量为核心，为病人供给优质服务的宗旨，深入开展了以病人为中心的健康教育，经过发放健康教育手册，以及经过护士的言传身教，让病人熟悉掌握疾病防治，康复及相关的医疗，护理及自我保健等知识。

2、八月份在三病区开展整体护理模式病房，三病区护士除了向病人讲授疾病的防治及康复知识外，还深入病房与病人谈心。

3、全年共收治了住院病人2357个，留观125个，手术室开展手术2380例。急诊护士为急诊病人供给了全程服务，包括护送病人去拍片，做b超、心电图，陪病人办入院手续，送病人到手术室，三个病区固定了职责护士、巡回护士，使病员得到了周到的服务。

4、全院护理人员撰写护理论文30篇，其中一篇参加了全国第三届骨科护理学术交流，有3篇参加台州地区学术交流，有2篇参加《当代护士》第二十一次全国护理学术交流。

七、存在问题：

1、个别护士素质不高，无菌观念不强。

2、整体护理尚处于摸索阶段，护理书写欠规范。

3、由于护理人员较少，基础护理不到位，如一级护理病人未能得到一级护士的服务，仅次于日常工作。

4、病房管理尚不尽人意。

一年来护理工作由于院长的重视、支持和帮忙，内强管理，外塑形象，在护理质量、职业道德建设上取得了必须成绩，但也存在必须缺点，有待进一步改善。

__年护理工作打算：

(一)继续提高护士长的管理水平和护士的业务水平。

1、请院领导组织护士长到兄弟单位参观学习，扩大知识面，吸取先进经验。

2、继续做好护士长手册的登记和考核，继续坚持夜查房制度。

3、鼓励护士长多写论文，参加省内外的护理论文交流，开阔眼界，提高管理本事。

4、进一步提高护士的业务素质。

1)继续抓好三基培训和专科护理理论，定期考核上半年各一次。

2)要求基础护理技术操作规范化，每月考一项操作，由护士长参加考核。

3)鼓励护士参加函授学习或自学考试，以提高理论知识。

4)鼓励护士多撰写论文。

(二)加强院内感染：严格按院内感染管理要求做。

1、坚持晨护一床一巾一消毒。

2、一人一针一管一带，用后及时毁形，浸泡消毒，集中处理。

3、消毒物品坚持包内放化学指示剂。

4、坚持床头桌每日消毒，一桌一巾。

5、病区坚持每日对治疗室、换药室、紫外线消毒，每周除尘，并有记录。

(三)加强护理文书管理，使护理文书合格率达二级甲等医院要求。

(四)狠抓护理安全：严格执行查对制度及各项操作规程，严防护理差错事故发生。
