乙方应向甲方提供职业病危害现状评价报告书8份。

五、双方义务与责任

1.甲方的义务与责任：

1.1 甲方应提供与评价项目相关的下列资料，并对所提供资料的可靠性和准确性负责：

1.1.1 建设项目总平面布置图、车间平面布置图。

1.1.2建设项目工艺流程简介和工艺流程图。

1.1.3原料辅料使用情况(组成、用量等)，生产工艺流程中存在化学物料名称和接触情况。

1.1.4建设项目设备情况。

1.1.5 建设项目职业卫生防护设施：如通风除尘排毒系统设计性能、车间通风换气(全面通风换气量、空气调节及洁净区参数)、事故应急求援设施(紧急救援站的面积、冲洗装置等)、污水处理系统等。

1.1.6 职业卫生管理措施：生产制度、劳动定员、安全操作规程、应急救援预案、作业工人的个人防护、年度职业病防治计划、培训制度、健康监护制度、有害因素监测制度等。

1.1.7 卫生辅助设施：工作场所办公室、更衣室、盥洗室(盥洗水龙头个数)、浴室(淋浴器的个数)、宿舍、食堂的设置情况。

1.2 根据本合同规定向乙方支付评价费用。

1.3 为乙方到现场工作的人员提供相应的必要的个人防护装备。

六、评价费用和支付方式

我是一个小学生。我过得很开心。我长着长长的头发还有一张可爱的脸蛋。但是我很矮。我是我们班最矮的学生，为此我感到很羞耻，但我的老师和同学都说我以后我会长高的来安慰我。除此之外，在学校里我是很开心的。有很多学生陪我玩，我学习很好。所以，回家后我的父母也很高兴。我爸爸经常带我去各种有趣的地方玩。我的妈妈经常给我做美味的食物。我真是太幸福了!

大家好!

我很荣幸能站在这里参加班长的竞选，在此我对大家的支持表示由衷的感谢。

首先，我向各位介绍一下自己：我叫。

我并没有诗人李白那长河之水天上来，奔流到海不复回的豪迈;也没有绝世伟人毛泽东那数风流人物，还看今朝的气魄。但寸有所长，尺有所短，天生我才必有用，我有年轻作为资本，有激情提供动力，有能力作为保证。

我敢爱敢恨、敢想敢做，我喜欢真心实意，厌恶勾心斗角，我崇尚高度与长度，鄙视懦弱与胆小，我有极大的自尊心、有强烈的责任感、有极强的团队精神与合作意识。小时候，父亲曾对我说过：做事要脚踏实地，一步一个脚印的去做。于是，我从一名中学生到今天走进大学一年级、走上这一神圣的地方，我深感机会的来之不易，需要好好珍惜。

正是由于这种动力，当我站在这里的时候，倍感自信。当然仅仅有自信是不够的，还要有一定的能力。而从小什么都不会到今天能独立策划活动，从不会到会，从不好到好的过程，就是凭借着自信和虚心的态度来认真完成每一件工作的。

在中学，我担任过班长、学生会委员。在工作中，我自觉培养了我的合作与协调能力，我能真诚的与别人合作，并且协调好同学们之间的关系，对不良行为敢于大胆管理，从不姑息纵容，因此，赢得了老师和同学们的好评。

我经常从同学的角度来看待我们班干部的问题。在平时多与同学们谈心、沟通、交流，了解他们的性格、思想、爱好，倾听他们的心声，充分的听取他们的建议和意见。

我曾从他们的角度来组织活动，这样班级活动也总是开展得热火朝天。但人非圣贤，孰能无过，我也犯过错，但我敢于面对自己的错误，从同学与老师的帮助中虚心的接受批评，并改正错误，而且决不充许自己第二次在同一地方跌倒。

今天我之所以在这里参加竞选。我希望把我们班建立成为学校领导与学生之间心灵的桥梁，成为师生的一条纽带，成为一个真正的为同学服务，同学们真正爱它的团体，同时也想锻炼一下自己的能力，给自己一个展示能力的舞台。

与他人相比，我觉得自己多了一份自信，因为我是一个不服输的人，我有着一颗不折不扣的责任心和对我们班的满腔热情。当然，我如果没有当选，我将一如继往的认真工作，并且会做的更好，全力支持新一届班委的工作。因为我爱这个团体，因为爱，所以我要竭尽全力的去做。

如果我有幸当选，我将实事求是的为同学服务，并将与风华正茂的同学，团结协作，与时俱进，大胆改革，充分发挥我们我们班应有的作用，让它成为名副其实的团体。

最后我还想说：这次参加竞选班长本身就是对我的锻炼和考验，如果我当选为班长，我愿当一名同学们的勤务员，尽职尽责地为同学们服务。在课余时间组织各类文艺活动，丰富同学们的校园生活。我只想有个机会，用爱使我们的校园生活变的更加美好，请大家支持我，请记住我的名字。

暑假飞逝而过，新的一学期也即将开始。这学期，我任教初一(8)班与初一(10)班的英语。为了把教学工作做好，打下良好的成绩基础，现特拟教学工作计划

一. 政治思想方面

坚持正确的政治方向，热爱教育教学工作，以教书育人、奉献教育为己任，严格执行党的教育教学法规，以高尚的师德标准来要求自己。

二. 业务方面

备课方面

认真搞好集体备课，集思广益，确定每单元讲课思路与方法。头一天备好第二天的课，备好教材、学生、教法、教具辅助手段等。教法的设定与学法的确定要体现以生为本，发挥学生的主导作用，使之真正成为学生的主人。

2.讲课方面

切实落实好任务教学，合理有层次设置各项任务，指导学生带着目标去完成任务，教师尽量少讲，在课堂上多给学生们实践的机会。教师要尽量使自己的课堂教学丰富多彩，增强趣味性，极大程度的去调动学生们的学习积极性。

3.批改方面

学生的作业布置实施分层布置，使之真正成为学生们巩固课堂所学的重要辅助手段。作业坚持天天批改，并写上有效的激励性语言，增强学生们的学习信心。对于一些成绩较差的学生，要搞好面批，并进行有效的学习方法指导和信心鼓励。

4.考试方面

每天坚持晨考，并及时进行复试，每单元学完之后进行知识点小考和单元测试，对于出现的问题及时进行讲解、训练与强化。

I’m very happy and I like to make friends with others. I also like singing but traveling is my favorite, I have been to many interesting places in China but I haven’t been to other countries. What a pity!

At first , you must read many articles and know many words, if you meet up with some new words, you can look them up in the dictionary, you should know their meanings, how to read them and spell them. If you keep working hard, you will be successful. Then, you ought to speak English as much as possible. Remember an old saying, Nothing is impossible in the world if you put your heart into it.
